About the position: The Per Diem HHA is required to work in different capacities as needed (i.e. working different shifts at the hospice house, in the field, or in the office). The HHA is responsible for observing the patient, reporting these observations and documenting observations and care performed. The hospice aide will work in the community home environment and will be assigned responsibilities in a manner that promotes quality, continuity and safety of a patient’s care.
Responsibilities:
· Performs personal care needs after given a patient’s assignment and attends to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately
· Meets safety needs of patient and uses equipment safely and properly (foot stools, side rails, O2)
· Gives personal care including baths, oral hygiene, shampoos and changing bed linen, and assistance with light laundry needs as assigned
· Assists in dressing and undressing patients as assigned
· Plans and prepares nutritious meals, and feeding the patient as assigned
· Takes and records oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency
· Provides proper care and observation of patient’s skin to prevent breakdown of tissue
· Reports on patient’s condition and significant changes and aware of the caregiver or other individuals living with the patient and interpersonal issues
· Assists in ambulation and exercise as instructed by the hospice nurse or therapist
· Offers and assists with bedpans and urinals
· Performs range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency
· Provides respite for patient’s/family/caregiver when on-site, as appropriate
· Keeps patient’s living area clean and orderly, as assigned
· Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct
· Participates in quality assessment performance improvement teams and activities
· Respectful of patient and family/caregiver environment and patient’s personal needs
· Documents HHA visits in EMR as per MH hospice standards and follows HHA care plan
· Attends Monthly HHA meetings and In-services
· Maintains required competencies and Licensure to perform job
· Attends IDT meeting when schedule allows
· Other duties as assigned
Qualifications:
· Current CA HHA license
· Current CPR certification
· One year experience as an HHA
· Valid California Driver’s License and an automobile that is insured in accordance with state requirements
· Computer proficiency in Microsoft Office and willingness to learn new software as needed
· Team player with excellent organizational, communication, and problem-solving skills
· Ability to read and document care that is given
· Ability to follow written and verbal instruction
· Demonstrates tact & patience
· Understands hospice philosophy & comfortable providing specialized care to the terminally ill
Physical abilities/requirements:
· Ability to safely lift/transfer 50 lbs with or without assisted devices (patients, supplies, etc)
· Ability to push, pivot, pull, and reposition 50 lbs with or without assisted devices
· Ability to bend, squat, stoop, crouch, kneel, reach, climb, and rotate
· Manual dexterity (grip/grasp/pinch)
· Ability to stand for prolonged periods of time
· Ability to walk for prolonged periods of time
· Ability to drive for prolonged periods of time
· Visual and hearing acuity with or without assisted devices
· Withstand different work environments
o Internal- office noise and interruptions, open workspaces
o External- a number of elements including but not limited to: various community & living environments, weather, & travel/traffic conditions
Please Note: The salary range provided here is only a reference guide, but final salary depends on work experience.
Pay: $22.50 - $29.46 per hour
