Work Hours/ Shifts
- Part- Time Days Monday-Friday 8am-12noon but can be flexible. Could do Monday, Wednesday, Friday.
- This person works with SN, PT and OT for bathing and personal care needs for our Home Health patients.
Location:
30-45 mile radius from hospital. Areas could include in Kansas: Wyandotte, Leavenworth, Douglass, Johnson, Miami, and Franklin counties. In Missouri: Clay, Cass, Jackson, and Platte counties.
The role you'll contribute to:
This position is accountable for assisting the health care professional by providing personal health care services and assistance with activities of daily living to the patient as specifically identified in a written assignment and necessary for the accomplishment of patient care plan goals.

- Follows appropriate safety measures when providing patient care.
- Provides weekend and holiday coverage as assigned by the patient care supervisor.
- Offers to take on other responsibilities to help the entire Home Health Care Team.
- Provides information for personnel file.
- Communicates pertinent information to the PCS, Case Manager and other team members.
- Attends in-service training, as scheduled, but at least 12 hours per year.
- Follows plan established on the HHA assignment sheet.
- Documentation clearly reflects HHA assignment.
- Documentation is accurate and follows established Care Plan.
- Participates actively in meetings, as scheduled.
- Mentors new employees, as requested.
- Follows frequency and duration of HHA services established on HHA care plan.
- Demonstrates the ability to organize workload in order to meet productivity standards.
- Complete documentation within 24 hours.
- Completes documentation accurately the first time.
- Responsibly cares for tools/equipment issued by agency.
- Kansas Home Health Certification (90 hours of training and 16 hours of supervised practical training by a qualified RN)
- BCLS obtained through the American Heart Association
- Valid Driver's License
- Current Car Insurance
- 1 to 2 Years of Position-Related Experience
- Preferably, have had at least one year's employment experience as a Nurse's Aide in an institutional setting.
- A minimum of 90 hours of training, including sixteen hours of supervised practical training by a qualified RN.
- Successful completion of a Home Health Aide competency evaluation.
- Good listening skills and ability to access desires of the patient in an accepting manner.
- Ability to function independently, problem-solve, prioritize and make decisions related to Home Health Aide responsibilities.
- Demonstrates sympathetic attitude toward the care of the sick and ability to deal effectively in a hostile or emotional patient situation.
- Effective interpersonal skills in relating to referral sources, co-workers and patients.
- Ability to communicate courteously with physician, clinical staff and patients.
- Knowledge and competency to perform required documentation and patient care procedures as a Home Health Aide.
- Demonstrates ability to read, write and carry out directions and to deal effectively with the demands of the job.
- Ability to use various tools, equipment and work aides as necessary to perform job duties.
- High School Diploma or Equivalent Preferred
- 5 Years of Position-Related Experience Preferred
