SUMMARY OF JOB:
Assumes responsibility for coordinating and facilitating Quality Improvement activities.

RECRUITMENT REQUIREMENTS:
- BSN from an accredited school of nursing required, MSN preferred.
- Minimum of five years experience in a healthcare setting, e.g., hospital, home health care, long-term care setting required.
- 3 year Home Care/hospice experience required
- 1 year of Quality Improvement experience in a healthcare setting preferred
- Knowledgeable of federal and state Home Care/hospice regulations.
- Knowledgeable of Home Care/hospice accreditation and compliance regulations and activities.
- Knowledgeable of Quality Improvement principles and activities.
- Proficient in Microsoft office and other PC applications.
- Experience in electronic medical record documentation preferred.
- Ability to perform Root Cause Analysis
- Good written and oral communication skills.
- Ability to travel to other locations as necessary.
LICENSE AND REGULATORY REQUIREMENTS:
- Current New Jersey or Pennsylvania RN license required.
- Valid driver’s license.
