Home Care Case Manager, SLC local

The Case Manager will work closely with caregivers, clients, and family members to deliver individualized care for our clients. You will assess our client’s needs, develop customized care plans, and work with our caregiving team to provide first-rate ongoing service. (We are looking for LOCAL Salt Lake County candidates with 3+ years of previous experience in case management. This is NOT an entry level position).

The Case Manager will report to our Agency Manager, work with our team in person at headquarters in Millcreek, Utah, and conduct in-home assessments throughout Salt Lake County.

Ideal applicants will have worked with seniors (personally and/or professionally) and have at least 3 years of experience working in Case Management in settings such as Home Health agencies, Hospice, Geriatrics, Social Work, or Care Coordination. Clinical skills for coaching caregivers and providing direct care training. Current CNA or willing to obtain a CNA. Soft sales experience to close the deal to enroll new clients.

Case Manager Responsibilities

• Communicate with potential clients and their families about Visiting Angels and assess needs.
• Conduct in-home assessments with prospective clients and their families to discuss how our services can assist their loved one in remaining safe and independent at home.
• Create custom care plans for each client.
• Collaborate with our scheduler to match caregivers with clients based on needs, skillsets, and personalities.
• Facilitate client and caregiver introductions to provide peace of mind to the client when allowing new caregivers into their home.
   

Requirements

• Three years of Case Manager/supervisory experience in elder care is required. This can be experience working in Home Health & Hospice, Social Work, or Care Coordination.
• A strong desire to build a client-focused, long-term career with Visiting Angels.
• Outstanding communication and interpersonal skills.
• Strong problem-solver with ability to work independently as well as part of a team.
• Highly proficient in modern software applications (Microsoft Word, Outlook, and Excel), and Google Meet, Gmail for business, apps, WellSky, etc.
• Education: Associate’s Degree; higher education preferred.
• A true self-starter, who can understand goals, prioritize work efficiently, and be flexible, resilient, calm, and maintain a sense of humor.
• Present a well-groomed professional appearance.
• Foster a culture of excellence within the company, aligned with our award-winning leadership and customer service.
• Valid driver's license with a clean MVR.
• Ability to lawfully work in the U.S.
   

Job Type: Full-time

Compensation: $25.00 - $28.00 per hour

Schedule:

• Monday to Friday 8:30 am - 5:00 pm in person
• On call rotation one week per month

Ability to commute/relocate:

• Salt Lake City, UT 84124.  Reliably commute to our office or planning to relocate to our service area in Salt Lake County before starting work.  (Required)

Experience:

• Case management: 3 years (Required)
• Hands-on experience providing personal care/direct care to someone older (personally or professionally)
• Sales experience to pitch services to potential clients and get them to enroll

License/Certification:

• Driver's License (Required)
• CNA, LVN, BSW (Preferred)
