Job Description 1. Direct Patient Care Participate in patient care rounds with the ID consult service and provide recommendations to optimize antibiotic usage Participate in targeted daily reviews of antibiotic usage as part of antibiotic stewardship activities Participate in reviews of positive blood culture results to ensure appropriate antibiotic therapy Promote appropriate antibiotic selection, dosing, monitoring.

Engage in therapeutic drug monitoring and pharmacokinetic consultations. Monitor conversion of intravenous to oral antibiotics. Review of medication profiles to assure optimal use of antibiotics while accounting for all co-morbid conditions Monitor drug therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, adverse drug reactions, appropriateness of drug and dose, and therapeutic duplication Optimizing antibiotic regimens to prevent and reduce medication errors Ensure safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols Provide patients with discharge counseling as applicable Facilitate discharge planning and transitions of care related to medication use when necessary Provide medication information to interdisciplinary medical teams Document pharmacotherapy interventions within the Electronic Medical Record Education responsibilities: Provide education to the ID team and house staff through both formal lectures and case-based education during clinical activities Facilitate, develop, and implement lectures to pharmacy staff in Infectious Disease related topics Coordinate education to pharmacists on appropriate antimicrobial selection.

Supervise and conduct continued education to the pharmacists and pharmacy students Serve as preceptor for pharmacy students on clinical rotations Provide in-services to nursing staff on pertinent pharmacy-nursing related topics Provide patient specific education to patients as requested by medical staff or nursing personnel Participate in hospital education programs Departmental responsibilities Serve as a resource to staff for the evaluation of appropriate antibiotic usage criteria. Develop policies or practices that encourage selection of the antibiotic regimen with the narrowest spectrum of activity based upon culture and sensitivity results. Continuously review drugs of choice and monitor utilization of antibiotic agents Prepare formulary presentations regarding new antimicrobial agents.

Develop programs that target high cost and/or specialized antimicrobials. Develop medication guidelines and protocols that match national and clinically appropriate treatment standards, while respecting the cost structure of the institution Pursue opportunities for optimization of Electronic Medical Record Coordinate the selection and safe use of alternative medication in response to drug shortages Support the pharmacy department's clinical endeavors Review the use of medication for compliance with pharmacoeconomic programs Participate in the review and approval process for non-formulary medication use requests. Suggest appropriate, cost effective therapeutic alternatives to medical staff when necessary.

Monitor cost of antibiotic usage and report to Section Chief and Pharmacy Leadership Provide accurate, adequate, and timely drug information to professional staff Contribute to the publishing of pharmacy newsletter Prepare and review drug use evaluation criteria, adverse drug reaction reports, and pharmacy interventions summary to various committees Participate in review and evaluation of medication events related to antibiotics Participate in quality improvement activities of the department Collect, analyze, and maintain data for all documented interventions Establish productive, collaborative relationships among the multidisciplinary healthcare team Sustain professional competence and development through participation in workshops, seminars, and professional organizations Complete special projects as assigned by Pharmacy Leadership Participate in Pharmacy and Therapeutics committee Support the antibiotic stewardship endeavors of the health system and participate in Enterprise Antimicrobial Stewardship Committee Assist in reporting antimicrobial usage and resistance patterns to NHSN Minimum Qualifications For Level I: 1. Possession of a valid NY State license to practice pharmacy. For Level II: In addition to meeting the qualification requirements of the lower level: 1.

Two years of post licensure experience; or 2. Completion of a post graduate specialty residency or fellowship program; or 3. Current Pharmacy Board Certification.

For Level III: In addition to meeting the qualification requirements of the lower levels, an additional: 1. One year of post licensure supervisory experience or specialty clinical experience.
