Who We Are:

Liberation Programs is one of Fairfield County's leading behavioral health organizations specializing in treatment for all types of substance use disorders including alcohol, opiates, depressants and stimulants. Our goal is to help individuals and families overcome addiction in order to restore their lives and ultimately strengthen our communities. Liberation provides services for youth, adults and families that include two inpatient treatment programs, outpatient and intensive outpatient services, health education for older adults and people living with HIV/AIDS and other chronic illnesses, treatment and resources for adolescents and their families, education and prevention efforts in the community, and permanent supportive housing for families. In operation since 1971, the agency has sites in Greenwich, Stamford, Norwalk and Bridgeport and served more than 2,400 individuals in our last fiscal year.

Our Mission:

Liberation Programs' mission is to provide prevention, treatment and recovery services to help individuals and their families impacted by substance use and mental health conditions to foster hope and maintain wellness.

We are seeking a highly motivated and compassionate individual to join our organization as a Mobile Ryan White Prevention and Outreach Specialist. In this role, you will be responsible for providing targeted HIV prevention education and Outreach services to at-risk populations within the community. The Mobile Ryan White Prevention and Outreach Specialist will play a vital role in promoting awareness, reducing stigma, and connecting individuals to essential resources and support services.

Key Responsibilities:

1. Conduct Outreach Activities: Plan and implement community outreach initiatives to engage at-risk populations, including individuals living with HIV/AIDS, people who inject drugs, sex workers, LGBTQ+ individuals, and other vulnerable groups. This may involve mobile outreach efforts, such as visiting community centers, clinics, shelters, or conducting street outreach.

2. HIV Prevention Education: Deliver comprehensive and culturally appropriate HIV prevention education sessions, workshops, and presentations to target populations. Provide information on safer sex practices, HIV testing and counseling, harm reduction strategies, and other relevant topics to promote healthy behaviors and reduce the risk of HIV transmission.

3. Resource Referral and Case Management: Assist individuals in accessing critical resources, including medical care, mental health services, substance abuse treatment, housing assistance, and other support services. Conduct comprehensive assessments, develop individualized care plans, and facilitate referrals to community-based organizations or healthcare providers as needed.

4. Conduct HIV Testing and Counseling: Administer HIV testing and counseling services, ensuring adherence to established protocols and confidentiality standards. Provide pre- and post-test counseling, deliver test results, and ensure appropriate referrals for follow-up care and support

5. Data Collection and Reporting: Maintain accurate and up-to-date documentation of all outreach activities, client interactions, and service delivery. Collect relevant data and prepare reports to monitor program outcomes, measure impact, and support grant reporting requirements.

6. Collaboration and Networking: Establish and maintain effective working relationships with community partners, healthcare providers, social service agencies, and other stakeholders. Collaborate on joint initiatives, participate in community meetings, and actively contribute to local HIV prevention efforts.

Qualifications and Skills:

- Bachelor's degree in public health, social work, community health, or a related field (Master's degree preferred)

- Strong knowledge of HIV/AIDS prevention, treatment, and care

- Experience working with at-risk populations, including individuals living with HIV/AIDS, people who inject drugs, sex workers, LGBTQ+ individuals, or other marginalized communities

- Familiarity with harm reduction principles and strategies

- Excellent communication and interpersonal skills, with the ability to engage diverse populations

- Demonstrated ability to work independently and as part of a team

- Proficiency in data collection and reporting

- Sensitivity to cultural, social, and economic diversity within the community

- Valid driver's license and ability to travel within the designated area
