HIV Medical Case Manager


Department: HIV Program

Job Status: Full Time

FLSA Status: Non-Exempt

Reports To: Medical Practice Association HIV Program Administrative Coordinator and Lead Case Manager

Grade/Level:

Amount of Travel Required: Minimal

Work Schedule: Monday-Friday; 8 am-5 pm (or similar)

POSITION SUMMARY

This position is an integral part of the Medical Case management team that provides support for individuals who are living with HIV/AIDS. Works closely with the primary health care providers, state, and local agencies. This position will connect clients to necessary services including, but not limited to: Ryan White programs, housing and patient assistance, dental services, transportation, and more. This position delivers HIV prevention education to clients as well as administers HIV rapid testing and counseling services. This position is funded through Ryan White program grants as well as through the Medical Practice Association.

ESSENTIAL FUNCTIONS

Essential Functions Statement(s)

Case Management

Enrolling Clients in Services

  • Provides patients with basic information about Ryan White programs (Parts B, C and D), ACA, patient assistance programs, housing, dental care, food banks, Medicaid and Medicare benefits, and other supportive services.
  • Assists clients in enrolling in the Affordable Care Act.
  • Performs complete social service needs evaluations on appropriate patients, including financial resources, support programs, transportation, nutritional assessments, etc.
  • Maintain accurate and complete records of case management activities.

Referral Services

  • Performs referrals based on evaluations of appropriate patients, including financial resources, support programs, transportation, nutritional assessments, etc.
  • Accesses pharmaceutical patient assistance programs and coordinates appropriate patient pharmaceutical care.
  • Coordinates services with local, state, and federal agencies.

Ryan White Part B Standards of Care

  • Communicates eligibility status to patients and external providers in a timely and accurate manner.
  • Timely and accurate documentation of patient/Case manager contacts.
  • Updates eligibility of all clients at least annually or as needed.
  • Adhere to the Kansas Department of Health and Environment case management standards of care
  • Attend state and local meetings as necessary.
  • Remain compliant with HIPAA regulations regarding patient confidentiality.

HIV Counseling and Testing

  • Provide updated prevention education information to patients.
  • Has knowledge of HIV treatment and medicine adherence practices and provides HIV testing and counseling as needed.

Collaborative Efforts

  • Works collaboratively with other members of the HIV Program team
  • Coordinates services between KUSM-W MPA HIV Program Providers' patients and regional Case Managers, and refers to other service providers and Part C contractors
  • Participate in community health fairs and other related events.
  • Serves on committees related to the Ryan White Program, KU, MPA, Midtown Clinic, State-wide, or others, as required by the position or through volunteer initiatives. Such events may include; World AIDS Day, patient holiday celebrations, community outreach activities, educational and training programs, or fundraising efforts.

Education: Bachelor's Degree in social work, counseling, nursing or other health-related area preferred

Experience: One to two years related experience in a full-time or part-time position. Internships or volunteer experience may be acceptable as well. Prior experience in a medical setting is desirable.


SKILLS & ABILITIES

Computer Skills

Basic knowledge of Microsoft Office Basic knowledge of database management. Ability to learn new databases (EMR and the KDHE client database).

Certificates & Licenses

Will be required to be certified as an HIV/AIDS Counselor and Educator.

Other Requirements

Effective interpersonal relationship/cross-cultural communication skills and strong social service orientation. Background in crisis intervention and substance abuse intervention. Knowledge of HIV/AIDS. Knowledgeable of and adherence to all standards for Universal Blood and Body Fluid precautions.
