SUMMARY: Performs all general clerical duties including sorting, pulling, searching charts for appointments, filing, retrieving, and answering phone calls. Ability to assume all job duties completed within the medical records department in order to cover for employees on vacation, LOA or out sick at all the clinic locations. Requires employee to work flexible hours without notice with starting time between 6:00am and 8:30am as deemed necessary by the supervisor and or department manager.

ESSENTIAL FUNCTIONS:
Essential functions are those tasks, duties and responsibilities that comprise the means of accomplishing the job’s purpose and objectives. Essential functions are critical or fundamental to the performance of the job. They are the major functions for which the person in the job is held accountable. Note: (other duties may be assigned, deleted or changed at any time, at the discretion of management, formally, or informally, either verbally or in writing).
1. Pull records for scheduled appointments, add-on appointments, telephone messages, special requests, pharmacy refills, etc. and deliver to appropriate provider or department.
2. Research patient medical record numbers; sort and file all incoming initialed reports in patient’s medical record folder (daily); send initialed paperwork to appropriate PCP for signature.
3. Retrieve scan in and refill all charts daily.
4. Search and re-search for missing charts or outstanding chart request according to departmental protocols.
5. Use CRT to research, track and issue out chart.
6. Answer telephone, take messages, and transfer calls to the appropriate department.
7. Assist nurses/providers with medical record issues and concerns.

EDUCATION and/or EXPERIENCE: High school diploma or general education degree (GED) required. One year related experience and/or training or equivalent combination of education and experience required.
