Job Summary
  • The HIM Physician Documentation Liaison is responsible to have an in-depth understanding of the Health Information Department policies and procedures, Medical Staff Rules and Rules pertaining to clinical documentation, CMS (medical records) and DNV/Joint Commission (medical records) and be able to fully function in all other aspects of HIM responsibilities. This position ensures the quality of the electronic and paper medical records by verifying their completeness, accuracy, and proper entry into available systems. The position requires constant collaboration with internal departments and physicians to ensure efficiency and precision related to clinical documentation in a timely manner. Assigned tasks will ensure legal and regulatory standards are completed for patients' health information including physicians' delinquent health records suspension process.

Education
  • High school graduate or equivalent required
  • Some college or AA degree strongly preferred.

Licenses and Certifications
  • Must successfully complete and maintain LA City Fire Card certification at the time of hire or within the first 30 days of employment.
  • RHIT or eligible strongly preferred.

Experience
  • Previous medical records experience in an acute care general hospital or studies in Health Information Technology required.
  • Some general education college courses are strongly preferred.
  • Excellent organizational skills with attention to detail required.
  • Excellent communication skills (patience, attentive, and ability to communicate clearly both verbally and in writing)
  • Customer-oriented attitude required.
  • Intermediate proficiency level with medical terminology is strongly preferred.
  • Intermediate Microsoft Suite (Outlook, Excel, and Word) experiencerequired.

Required Skills
  • Meticulously audit inpatient and outpatient medical records
    • Notify physicians and providers daily/weekly of their incomplete/delinquent medical records.
    • Notify physicians of potential suspension
    • Notify physicians of suspension as well as any resulting disciplinary actions as mandated by VPH Medical Staff Rules and Regulations
  • Analyze discharges of all patient types in order to identify physician deficiencies according to HIM policies and procedures.
    • Clear physician deficiencies in the electronic health system
  • Performs self-quality checks on all work to assure accuracy prior to informing physicians and providers of missing or incomplete reports.
  • Provide excellent customer service by being attentive and respectful; ensures understanding of the legal health record and follow through as promised; and being proactive in identifying concerns or problems.
  • Utilize electronic health information systems to obtain information related to patient encounters
  • Release confidential patient information to external healthcare providers as governed by state and federal laws.
  • Process multiple medical record numbers and merging of medical record numbers.
  • Process rejected physician's orders and assign the orders to the correct physician in the EHR.
  • Complete birth certificates and assist in the completion of declaration of paternity forms if needed.
  • Register and assist patients in Hospital Patient Portal
  • Monitor the mail in the automatic email/fax system, Forward Advantage
  • Answer all incoming telephone calls by the second ring. Check voicemail messages throughout the day and return all calls within 2 hours.
  • Assist with patient-requested amendments to medical records in compliance with the Health Insurance Portability and Accountability Act (HIPAA)
  • Maintains current knowledge related to applicable statutes, regulations, guidelines and standards necessary to perform job duties in accordance with the requirements of the Corporate Compliance Plan. Complies with the requirements of the Code of Conduct, Corporate Compliance Plan and Compliance Policies and Procedures, including training requirements. Participates in compliance activities under the direction of the Department Supervisor, Department Director, and Corporate Compliance Officer.
  • Perform other related duties as assigned.

  • Demonstrates knowledge of and follows safety practices. Understands the importance of safety, including patient safety in the workplace. Maintains a safe environment for self and others.
  • Actively participates in the Patient Safety Program, including event reporting. Identifies sentinel events/near misses and responds per defined organization processes. Participates in education activities and process implementation. Demonstrates advocacy for the patient/customer and appropriately acknowledges patients, customers, and visitors.

  • Leads planning and implementation of educational assignments in the LMS.
  • Provides system optimization analysis and provides feedback and recommendations to stakeholders.
  • Oversees education partners' exemption rights, administrative privileges, reporting rights specific to unit, checklist evaluation/evaluator updates, and content removal/addition.
  • Produces accurate and timely regulatory completion reports to support education initiatives.
  • Assigns education per established schedules to meet regulatory compliance standards.
  • Performs technical functions in the LMS for registration, online resources, presentations, in-service education, evaluations, tests, tracking, and generation of reports.
  • Maintains education class calendars and assignments within the LMS ensuring accuracy.
  • Tracks assignments, milestones, and deliverables.
  • Escalates issues, as appropriate.
  • Serves as educational project intake coordinator for tracking and prioritization of assignments.
  • Confirms relevant product performance by designing and running reports.
  • Performs special duties, tasks, or assignments as required.
  • Provides independent oversight for activities associated with student onboarding.
  • Completes system access and security badge requests.
  • Schedules computer training and post-conferences.
  • Ensures all student onboarding materials are received.
  • Maintains records per required guidelines.
  • Acts as a liaison between affiliated schools, clinical instructors, hospital leadership, and students.
  • Maintains accurate community benefit data for all student specialties and submits reports in a timely manner.
  • Maintains accurate CMS data for vaccination status of all students completing education hours on campus.
  • Oversees affiliation agreements in collaboration with the hospital's Contract Administrator.
  • Interacts with vendors to provide information and resolve issues.

Salary Range: $23.04 - $30.06 per hour
