The RN would work with the family health centers and the mammography department at St. Elizabeth and to identify patients with a high Tyrer-Cuzick score (>20%) and/or family history of breast or ovarian cancer, and/or a known genetic mutation for developing cancer.  The RN would also provide direct patient support by providing but not limited to the following: comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with clinical care team, increase patient and provider satisfaction, increase patient quality, maintain point of contact with providers and serve as the first point of contact for patients and families.

  • Identify patients that would benefit from our services.
  • Provide clinical resources – where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information. 
  • Assess patients for psychosocial distress and referral to a medical social worker or counselor if more intensive support is needed. 
  • Answer questions as appropriate and serve as a liaison to the physician for additional questions and concerns. 
  • Address tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more. They refer patients to the appropriate contact, such as a social worker. 
  • Ensure any health information provided is accessible: this may include securing documents printed in the patient’s own language or arranging for a certified medical interpretation, either in-person or by phone/video conference. 
  • Function as a member of an interdisciplinary team to assess, secure and coordinate appropriate services through the continuum of care.
  • Empower patients, families, and caregivers to assume as much responsibility for their care as possible, with the constraints of age, illness and other factors.
  • In consultations with other members of the healthcare team, promote awareness of clinical trials as appropriate. 
  • Support the timely scheduling of appointments, diagnostic testing, and procedures.
  • Educate/identify patients for genetic counseling or fertility preservation, and facilitate appropriate referrals. 

 


Qualifications

Qualification

RN Required, BSN Preferred

A current RN license to practice in the state is required. If working in Oncology, preference for oncology to have oncology certified nurse (OCN)

BLS Certification throuhg the American Heart Association

 


Pay Range
$28.94 - $41.97 /hour
