Job Description
The High-Fidelity Wraparound Youth Support Partner works closely with the facilitator to support positive outcomes for the youth and families. This support person is with a graduate of wraparound himself or herself or was a youth with complex emotional or behavioral needs.
Requirements:Position Requirements
· Must be over the age of 21 and up to the 32 years old
· Must have lived experience as a child or adolescent who had mental, or substance use disorder
· Experience in navigating the child and family serving systems.
· Must have a Bachelor’s degree in a Human Service field from an accredited University and one year of experience working with the target population; or associate’s degree in a human service field from an accredited school and two years of experience working with children/adolescent/transition age youth.
Responsibilities and Duties:
· Passionate about applying the principles of High-Fidelity Wraparound, including Family Voice & Choice, Team Based Planning, Natural Supports, Collaboration, Strengths Based approach, and more
· Family and youth engagement strategies
· System of Care principles include supporting family driven care and interagency coordination.
· Monitors and documents the status of the youth and family’s progress and effectiveness of the strategies and interventions outlines in the Plan of Care.
