Your Choice At Home is an affiliate of Cobble Hill Life Center and a Certified Home Health Agency (CHHA). We're culturally sensitive, and many of our staff come from within the community that they serve. Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services. As the HHA Coordinator, you will be responsible for coordinating multidisciplinary home health services for patients in the community.
Schedule: Full-Time, M-F, 9am to 5pm

Qualifications:
1. High school diploma
2. Associate degree preferred
3. Good communication skills
4. Good typing skills
5. Experience working in a computerized environment.
6. Absolute integrity and sufficient maturity to handle confidential aspects of work
7. Experience with HHAeXchange required.
Physical Requirements:
1. Possess the sight/hearing senses or uses prosthetics that will enable theses senses to function adequately so that the requirements of this position can be fully met.
2. Must meet the regulatory health status requirements of the NY State Department of Health
Work Requirements:
1. Work in the office environment with regular exposure to Administration, Management, office staff, vendor agency's personnel, patients, and their significant others.
2. Must be able to sit for prolong period, transport materials or equipment, lift, move or carry some items throughout the day.

Job Functions and responsibilities:
1. Maintain HHAeXchange (home health aide vendor) system:
a. Broadcast HHA cases for staffing
b. Update HHAX with patient demographics, authorization, and recertification information
c. Collaborate with lhcsa (licensed home care agency) coordinators to ensure scheduling, billing, and authorizations are accurate.
d. Collaborate with office/field staff with any changes to HHA patient's care (e.g. hospitalizations, holds, discharges, emergency health concerns)
e. Troubleshoot lhcsa vendor issues.
f. Manage HHAX communications.
g. Update HHAX with provider rate increases
h. Notify lhcsa vendor of issues regarding aide services.
i. Process un-billing requests
j. Run weekly expiring authorization reports to ensure services continue in accordance with the POC and NYS Department of Health regulations.
2. Maintains Careficient (patient care) software system:
a. Assign HHA vendors to a patient schedule.
b. Collaborate with the nursing supervisor and nursing staff to ensure accurate HHA information reflected on 485, HHA POC, and care needs.
c. Run missed visit reports, remove missed visits from patient's schedule, scan and uploads missed visit notes from HHAX to CE
3. Send weekly HHA expiring authorization emails to intake staff and nursing staff.
4. Monitors HHA data entry specialist to ensure timely processing of duty sheets in accordance with YCAH billing policy.
5. Participates in Performance Improvement projects.
6. Accepts and completes assignments as given within scheduled time frame.
7. Projects positive business-like demeanor. Promotes positive work environment.
8. Maintains confidentiality of patients' records
9. Accountable - maintains an acceptable record of attendance.
10. Complies with policies and guidelines established for meal periods, breaks, sick calls, requests for time off and dress code.
