HEDIS Coordinator

Title: Patient Care Advocate

Duration: 3 months contract to hire

Target Start Date (based on successful onboarding completion): 05/01/24

Shift: 8:00 AM - 5:00 PM - Monday - Friday

Location: REMOTE - West Pennsylvania

Member-Facing / Traveling 3+ days of the week. Candidate must be located in the Western part of the state of Pennsylvania:

Walk me through the day-to-day responsibilities of this the role and a description of the project:

• Telephonic outreach to members, addressing gaps in care, and barriers to care i.e., transportation, offering in-home test kits for gap closures.

Describe the performance expectations/metrics for this individual and their team:

• 20 unique members per day, 3 calls per member at different times of day, 5 kits requested and returned per month, and 5 PCP appointments scheduled per month. Meeting and exceeding eighty percent (80%) for quality call reviews. Reach rate of 40%, engagement rate 20% of reached members. Effective utilization of Jaber and Nice Engage.

• Outreach members for gaps in care and/or additional training.

What previous job titles or background work will be in this role?

• Health Coach, Care Engagement Specialist, Nursing. HEDIS metrics, Knowledge of Quality Outreach platforms, self-motivated, team player, medical knowledge.

Required Skills/Experience/Education:

• 2+ years of RECENT Member Outreach, HEDIS and Motivational Interviewing Experience
• Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related health field
• Technology skills, Customer service skills and Ability to multi-task

Job Type: Contract

Salary: $30.00 per hour
