Job Summary
The senior Heart Failure Coordinator is the clinical team lead of the ambulatory Heart Failure program and is responsible for collaborating with the healthcare team to develop theory-based patient care for those with advanced Heart Failure from being referred through advanced treatment options and management.
Key Accountabilities
Fosters a "Just" culture among the Nursing and Medical Assistant staff
Collaborates with the health care team to develop theory-based patient care for those patients with advanced Heart Failure from referral through long term advanced therapies and medical management to ensure patient success
Exercises independent judgement and advanced clinical skills to provide nursing care and care coordination to these specified patient populations
Collaborates with CHF outpatient APP lead and Pharm D to standardize processes around triage, remote monitoring, GDMT escalation, and patient Heart Failure education
Establishes nursing leads for subspecialty Heart Failure clinics (HCM, Amyloid, HFpEF)
Assists and collaborates with the management of maintaining statistical information for departmental productivity reports as well as to maintain cost-effective unit operations
Assists the Administrator/Supervisor and Heart Failure Center of Excellence with interviews and makes recommendations for hiring new personnel, assigns work to and evaluates work performances of team members supervised to ensure high standards of care and skilled staff
Coordinates training and education of other team members as evidenced by training logs, competency documentation records, and team member education records
Participates and coordinates Heart Failure Coordinators involvement in Quality, Growth and Strategy, and research initiatives
Responsible for establishing, maintaining, and updating workflows for Medical Assistants and Heart Failure Coordinators in collaboration with clinic leadership
Ensures clinic operation workflows are adhered to by Medical Assistants and Heart Failure Coordinators
Education:
Graduate of accredited school of nursing and/or Bachelor's Degree in Nursing (BSN)
License/Registration - Issued by Florida or Compact State
Active RN License
CHFN (Certified Heart Failure Nurse), HFSA Heart Failure Certification (HF-Cert), or another advanced nursing certification required within 2 years of accepting position
Work Experience and Additional Information
5 years nursing experience in an acute/critical care/ambulatory setting
