The Position:
St. Patrick Center is hiring a Healthcare Navigator due to our agency expanding and so is our Support Services for Veteran Families (SSVF) program. The Healthcare Navigator will provide assessment, case management and care coordination, health education, interdisciplinary collaboration, coordination, consultation and administrative duties. The case manager will work closely with the Veteran's primary care provider and members of the Veteran's assigned interdisciplinary treatment team. Moreover, the healthcare navigator must have an interest in helping veterans reach their full potential. This position works Monday-Friday, 8:00 AM-4:30 PM.
