Primary Responsibilities:
The Health & Wellness Life Coachis a direct service delivery position responsible for motivating, coaching, and providing client-directed interventions aimed at helping participants find success and satisfaction within their significant life areas, including housing, careers, relationships, and health. The Life Coach will help participants find their motivation, recognize their skills, focus on their goals, and move past challenges that stand in the way. The Life Coach will participate as part of the Housing First Team as well as the adult Community Support Services Team. The Life Coach will assist the Housing First Team in upholding the five supportive housing principles and ensuring the targeted population is screened in for support and services.
Essential Job Functions:
- Work with clients to improve health and wellness as part of a person-centered treatment plan focused on whole-person strengths, needs, and life domains.
- Encourage positive behavior changes across multiple life domains including but not limited to housing, smoking cessation, stress management, physical fitness, and nutrition.
- Use client-directed approach for health coaching based on motivational interviewing techniques to help clients develop strategies to change their behavior that are personally meaningful and self-directed.
- Provide health coaching that is focused on the here and now, helping clients identify their current strengths to achieve their behavior-change goals.
- Provide direct client services according to ELC and grant performance standards.
- Ensure that referrals for services are acted upon in a timely manner.
- Work as part of an interdisciplinary treatment team and client/family-centered treatment plan that includes client needs, strengths, preferences, and services.
- Monitor and provide timely and accurate documentation of all client services
- Assist clients with accessing new resources and attempt and practice new skills in the community.
- Provide client transportation as relevant to the Housing First program and treatment plan.
- Work closely with community and agency partners and maintain a professional working relationship with these providers.
- Full understanding of HIPAA privacy laws and maintain client confidentiality.
- Provide coverage for crisis intervention services which may include evening or weekend hours.
- Other duties as reasonably assigned by the CSS Director.
EDUCATION and/or EXPERIENCE:
- Bachelor’s degree or equivalent experience necessary to meet State of Kansas licensing regulations
- Possesses the minimum qualifications as documented in the Kansas Case Management Program Standards
- Able to achieve certification with the Health & Wellness Life Coach through InHealth Lifestyle Therapeutics Coaching Academy (training provided)
- Able to complete all required trainings within six months of employment
- Familiarity with mental health theory and practice
- Clean driving record
- Incumbent must pass a security check as required by Medicaid Regulations
- Relates a positive approach to complex mental health issues
- Ability to communicate effectively orally and in writing
- Ability to establish effective working relationships with staff, community agencies, and consumers
- Ability to be sensitive to cultural and ethnic differences
- Ability to safely perform major job responsibilities with or without reasonable accommodations
