As an integral member of the Wellness team, the Health Coach works in partnership with primary care physicians and wellness service providers to help patients improve their health and wellness through sustainable behavior change.
The Health Coach uses a thoughtful and strategic combination of health education, health coaching, and behavioral analysis to help patients overcome barriers, increase health and wellness knowledge, and build habits to support or, improved health and quality of life. Daily tasks include performing one on one coaching sessions with patients, charting, and care collaboration.
ESSENTIAL JOB FUNCTIONS
Serve as a primary point-of-contact for patients. Provide one-on-one health coaching, education, and support through virtual visits and various modes of communication
evaluate health needs of patients, set achievable goals, and create tailored wellness plans.
Monitor patient progress, send follow up communication, adjust wellness plans as needed, and support patient connection to alternative care services and resources as appropriate.
Perform and comply with clinic policies, general health coaching procedures, and clinical program tasks, including adherence to charting, documentation and escalation protocols
translate scientific language and concepts into understandable, digestible information for patients.
Consult with primary care physician and fellow care team to promote high quality, patient- centered care.
Support the development of culturally and linguistically competent health education material.
Collect patient feedback and analyze data to supportprogram evaluation and improvement of coaching operations.
Attend all required meetings.
Complete tasks assigned by leadership and provide timely updates as needed.
( if applicable) In addition to the functions listed above, Health Coaches with active Registered Dietitian certification may perform the following additional essential job functions:
Create nutrition focused content for clinical programs. Provide one-on-one nutrition coaching for patients
QUALIFICATIONS AND REQUIREMENTS
Bachelors degree and 5+ years of relevant experience in health education, health promotion,
public health, exercise physiology, nutrition or related field.
Certification from accredited health coaching program required Board certification in health and wellness coaching (NBC-HWC) strongly preferred. Thorough knowledge of health behavior, learning theories and their application in coaching, quality of care and patient experience.
Demonstrated experience in health, nutrition, physical activity, and chronic disease education.
If applicable, Registered Dietitian with American Dietetics Association(required only as indicated above for additional job functions).
Compliance with AC Wellness Occupational Health Policy, including required immunizations.
Reside in California and has the ability to have a secure, private location with Wifi internet connection remotelyto perform all job responsibilities.
Excellent verbal and written communication skills
Comfortable with change and willing to work in a non-traditional practice model.
Excellent interpersonal skills and enjoy collaborating with team members.
