Specific Responsibilities:

This is a Health Information Specialist position responsible for collecting, coding, billing, retrieving and document medical records. Work is performed under the supervision of the Health Information Manager-SES. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions.  This position is responsible for ensuring services coded are adequately documented in the medical record to support the quality assurance activities, trend analysis and billing. Verify codes prior to entering in the Health Management System and submit to the billing unit based on Federal and State third party compliance guidelines.

 

This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. This position has the responsibility to view confidential information. Confidential Data Sets: Clinical Information Data Sets, Billing Data Sets, Social Security Data Sets, Cash drawer for all FDOH - CHD sites and Administrative Data Sets, Health Management System (HMS).

 

Responsibilities include analysis of medical documentation and coding of the diagnoses and procedures within the medical record and on data entry. Data entry diagnostic and procedural codes in the HMS billing system and record any documented HMC codes recorded on the encounter slips.

 

Translate the medical documentation to a numerical or alphanumerical national standard coding system. Compare medical

documentation and select the correct ICD-9-CM/ICD-10-CM code that supports the documentation within the medical record. Verify the accuracy of the code following established coding practices. Verify the EIM level of service code utilizing the Medicare and AMA guidelines and select the correct code based on the documented level of service. Refer to the Medicare and Medicaid program parameters for third party reimbursement prior to entry in the billing system. Apply all National coding guidelines to the practice of diagnostic and procedural coding.

 

Data Entry of ICD-10-CM, HCPCS and CPT codes:

Data entry of the diagnosis's codes, procedure codes, local codes, HMC codes into the billing application of HMS.

 

Verify insurance and return to AA or Clerical Supervisor for correction of insurance information.

Communicates with the billing staff regarding error on codes and fees not in the computer.

 

Reconciliation

Electronic reconcile each account and ensure the information has been accurately entered into the HMS system.

 

Consultation

Provide consultation with providers. Discuss documentation standards and terminology with providers if there is a need for clarification, correction or additional supporting data as needed. Provide ongoing training to providers through communication of new and changing regulations and procedures.

 

Denied claims, back billing and retro billing, reviewed and corrected. Verify information entered in the billing system. Verify the

diagnoses (es) codes are accurate.

 

Performs other duties as assigned.

 

Required Knowledge, Skills, and Abilities:

  • Knowledge of the procedures for securing, accumulating, processing, preserving, retrieving and controlling the distribution of medical records.
  • Knowledge of the procedures for coding, classifying, indexing and filing medical records manually and/or electronically.
  • Ability to use a nomenclature and classification system.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to communicate effectively.
  • Ability to establish and maintain effective working relationships with others
  • Knowledge of Medical terminology, ICD-9-CM/ICD-10-CM and CPT coding proficient, Federal and State Third Party Coding Guidelines.
