Form of workAs our Home Health Care Coordinator for King County, you will help our most vulnerable community members by assessing, facilitating and planning delivery of care so they can reach their desired health outcomes.
Every day you will manage a caseload and outreach of patients for King County. You will be expected to work with a multidisciplinary team to develop person centered goals for community members and find resources that will help obtain those outcomes.

The Home Health Care Coordinator is mainly focused on supporting King County. So you will have a mix of working in the field and remote.

- Complete member screening within scope of experience, training, and expertise, and communicates social, emotional and patient/family stressors to interdisciplinary healthcare provider team as relates to member’s plan of care.
- Coordinate with the member/family, the interdisciplinary healthcare provider team, insurance payers and community resources in organizing the outpatient care; promotes and facilitates effective chronic disease self-management and provides tools to assist members/families in achieving maximum levels of wellness and independence.
- Research and identify community resources, vendors, medical supply companies, healthcare agencies and other resources appropriate to the patient’s individualized plan of care.
- Bachelor’s Degree In Social Work, Psychology , Nursing or related Fiend
- 1 Years Of Related Work Experience
- Eligible for Agency Affiliated Counselor prior to date of hire and credential obtained within 60 days of hire.
- Current healthcare provider BLS certification.
