As a Health Guide Associate, you will:
- Schedule appointments with Health Guide (Nurse Practitioner)
- Prep medical charts: entering in medications, allergies, past medical, past surgical, family history
- Request current medication list from pharmacies
- Venous blood draw in office or at patient's home
- Order DME (glucometer, nebulizer, medical supplies)
- Coordinate individual's transportation to outside lab
- Send referrals and schedule appointments with outside providers (pcp, behavioral health)
- Support individual in continuity of care
- Follow up with individual prior to appointments to ensure they keep appointments
- Follow-up with patients after their appointments and update chart of any treatment changes (medications, upcoming procedures)
- Support Collaborative Practice between Health Guide (Nurse Practitioner) and Physician by assisting in inputting of data to google excel
- Print firsthand referral letters
- Role will be hybrid with 4 days in the Chattanooga office and 1 day work from home
You will be good if you have:
- Passion for empowering individuals with serious mental illness, substance use disorder, and complex medical conditions
- The ability to maintain schedule, prioritize, and/or escalate series of tasks accordingly
- Are self-directed and committed to follow-up and follow-through
- Comfort in navigating multiple technology systems including but not limited to Google Suite, EMRs, slack or microsoft teams.
The experience you bring to this role includes:
- 3+ years' experience in primary or behavioral health
- Medical Assistant (MA) or Certified Nursing Assistant (CNA) or Licensed Practical Nurse (LPN)
$35,000 - $40,000 a year
This is a full-time, exempt, salaried position reporting to firsthand's Health Guide. firsthand cares deeply about ensuring pay equity with every team member who joins. Commensurate on candidate experience, the expected range for this role's base salary is $35,000-$40,000.
