Overview
As a vital member of our team, you will play a pivotal role in optimizing patient care and outcomes, ensuring healthcare equity. You will be responsible for assessing needs, coordinating conversations, and customizing plans that lead to tangible impact in healthcare equity.
The Health Equity RN drives transformative change within Children's Mercy by leveraging technology and collaborative partnerships. This role is a key catalyst in ensuring equitable journeys for our patients and families by mitigating and escalating barriers to patient care. Through vigilant observation of real-time data and predictive insights in our Patient Progression Hub (PPH) this role will empower hospital and ambulatory teams with actionable information to enhance care delivery. By visualizing information on tiles and corresponding alerts, you will guide informed decisions that directly impact patient and family well-being. This role will also be responsible for seeking qualitative data, gathered through patient and family insights to understand and intervene as it relates to utilization patterns, uncovering underlying barriers, preferences and perceptions to ensure the right care, in the right place, at the right time. Embracing technology as a tool for positive change, the Health Equity RN will identify and dismantle barriers that create care interruptions. Dedication to incorporating innovation into patient care will drive continuous improvements in outcomes and experiences within this role. In addition to operational contributions, this role will collaborate under the guidance of Care Management, and in partnership with hospital leadership to foster equity and inclusion within programs throughout the organization. Through building relationships and facilitating conversations, you will promote Health Equity and address social determinants of health.
This is a day-time role in the Adele Hall, newly finished state-of-the-art Patient Progression Hub.

Responsibilities
Manages, expedites, prioritizes, and mediates clinical issues, removes barriers to patient care and addresses delays in care in real time assuring sound decision support.
  • Participates in Patient Progression and Care Management huddles and rounding daily.
  • Connects with local care teams to resolve barriers; collaborates in a supportive partnership.
  • Promotes collaboration and coordination between care teams to address barriers to patient care including language barriers, system barriers, and barriers related to social determinants of health.
  • Triages alerts and manages deviations from established patient care pathways; maintains ongoing situational awareness via investigation of the electronic medical record and additional clinical resources; responds, or coordinates response as necessary.

Leads the identification, reporting and management of barriers to patient care leading to optimal patient outcomes.
  • Utilize available IS systems and data within the Patient Progression Hub to perform analysis and prepare metrics for leadership.
  • Interface with Patient Progression Hub and Care Management leadership to discuss barriers to patient care and healthcare equity concerns.
  • Flag trends and present findings to leadership on regular cadence.
  • Lead and participate in assessments, data analysis and uses results to identify key process improvement opportunities.
  • Educates and collaborates with care teams, patients and families regarding enhancement in patient care.

Role models leadership and participates in ongoing professional development activities, making a meaningful contribution to quality improvement and continuing education of self and others.
  • Assumes leadership role in the clinical setting.
  • Provides clinical consultation to physicians and Children's Mercy team members regarding coordination of care, case management, and the continuum of care issues in a timely, positive manner.
  • Seeks consultation from appropriate disciplines/departments as required to expedite care and facilitate timely discharges.
  • Demonstrate responsibility, efficiency, good organizational skills, and effective communication skills with internal and external customers.
  • Responds to all requests appropriately, accurately, and in a timely manner.
  • Actively engage in the Children's Mercy Lean System including participation in huddles, problem-solving methodology, and submission of data as requested.
  • Regularly attends and contributes to the success of department meetings, committees, and other CM committee assignments.
  • Leads the application of new knowledge and innovation to advance improvements

Qualifications
  • Bachelor's Degree, BSN - Nursing, and 3-5 years' experience. Direct patient care, previous pediatric clinical experience and ability to navigate EMR and use software program. Public Health experience is preferred OR
  • Master's Degree, Nursing and 1-2 years' experience. Direct patient care, previous pediatric clinical experience and ability to navigate EMR and use software program. Public Health experience is preferred.
  • One of the following: Licensed RN - MO, Registered Nurse Multistate License Missouri required upon hire OR
  • One of the following: Licensed RN - Kansas, Registered Nurse Multistate License Kansas required upon hire OR
  • RN Compact license for all states besides KS and MO
  • Refer to Nursing and Advanced Practice Provider Licensure and Certification policy for required Life Support Certifications Required Upon Hire

Starting Pay
Our pay ranges are market competitive. The pay range for this job begins at $33.95/hr, but your salaried offer will be determined based on your education and experience.
Remote Work/Work from Home
This position is not eligible work remotely, which means that the person hired will be required to work onsite at one of our Children's Mercy locations and may not work from home.
