Job description

PRINCIPAL RESPONSIBILITIES:

Under the general direction of the Manager, Population Health and Equity, this position is responsible for helping to plan, organize, and implement Alliance Health Education and Cultural and Linguistic programs.

Principal responsibilities include:

  • Facilitate member registration for Health Education programs, including group and individual classes, case management, and other types of programs.
  • Maintain resource directory and referral lists of classes available to members in order to provide best geographic, linguistic, cultural and content match based on member need.
  • Conduct reminder calls and reminder mailings to members signed up for Health Education programs.
  • Coordinate Health Education and Cultural and Linguistic projects.
  • Respond to members, providers and internal Alliance staff requests for Health Education materials such as written materials, videos, and supportive items (medical alert bracelets, cookbooks etc.).
  • Conduct follow-up to members who have participated in Health Education programs to determine program effectiveness.
  • Represent the Alliance at community meetings and events.
  • Staff Health Education programs such as workshops, assist department with program development and implementation.
  • Track member attendance at Health Education programs and distribution of Health Education materials.
  • Create and run periodic departmental reports.
  • Administer payment of Health Education invoices and assist with monitoring department budget by entering expenditures into database.
  • Assist with development of Health Education and Cultural and Linguistic materials.
  • Assist with selection, review process, tracking, and ordering of written materials.
  • Coordinate distribution of Health Education and Cultural and Linguistic materials, including but not limited to: copying, printing of labels, and coordination with the mail house/mail room.
  • Arrange logistics for Health Education and Cultural and Linguistic projects including location, refreshments, supplies, and handouts.
  • Ensure CAC meetings are scheduled, and committee agendas are developed with the input of CAC members.
  • Maintain CAC membership, including outreach, recruitment, and onboarding of new members, that is adequate to carry out the duties of the CAC in a collaborative effort with the Communications & Outreach team.
  • Actively facilitating communications and connections between CAC and Contractor leadership, including ensuring CAC members are informed of Contractor decisions relevant to the work of the CAC.
  • Ensure that CAC meetings, including necessary facilities, materials, and other components are accessible to all participants and that appropriate accommodations are provided to allow all attending the meeting, including, but not limited to, accessibility for the individuals with a disability or LEP Members to effectively communicate and participate in CAC meetings.
  • Ensure compliance with all CAC reporting and public posting requirements.
  • Complete other duties and special projects as assigned.

ESSENTIAL FUNCTIONS OF THE JOB

  • Telephone contact with members and providers, providing assistance in accessing Health Education services.
  • Coordinate logistics for Health Education programs.
  • Assist with development, implementation, and evaluation of programs.
  • Staff Health Education programs.
  • Attend meetings with other department staff to identify on-going member concerns and design future outreach programs.
  • Assist with development and production of Health Education materials.
  • Contact with community organizations in coordinating Health Education services.
  • Comply with the organization’s Code of Conduct, all regulatory and contractual requirements, organizational policies, procedures, and internal controls.

PHYSICAL REQUIREMENTS

  • Constant and close visual work at desk or computer.
  • Constant sitting and working at desk.
  • Constant data entry using keyboard and/or mouse.
  • Frequent use of telephone headset.
  • Constant verbal and written communication with staff and other business associates by telephone, correspondence, or in person.
  • Frequent lifting of folders and other objects weighing between 0 and 30 lbs.
  • Frequent walking and standing.
  • Occasional driving of automobiles.

MINIMUM QUALIFICATIONS:

EDUCATION OR TRAINING EQUIVALENT TO:

  • Bachelor’s degree or equivalent work experience required.

MINIMUM YEARS OF ADDITIONAL RELATED EXPERIENCE:

  • Minimum two years administrative support experience.
  • Minimum one year managed care experience preferred.

SPECIAL QUALIFICATIONS (SKILLS, ABILITIES, LICENSE):

  • Fluency in Spanish preferred.
  • Proficiency in correct English usage, grammar, and punctuation required.
  • Excellent problem solving skills.
  • Ability to manage multiple tasks simultaneously.
  • Ability to work with members in a professional, respectful and responsive manner.
  • Ability to work with diverse groups including low-income populations and health care providers.
  • Knowledge of Medi-Cal and other entitlement programs desirable.
  • Knowledge of Alameda County communities.
  • Knowledge of Health Education principles and methods.
  • Ability to maintain cooperative relations with community and professional groups.
  • Ability to communicate effectively, both verbally and in writing.
  • Must be organized, detail oriented, and have the ability to work in cooperation with others.
  • Strongly proficient in Microsoft Word, Excel and Adobe Acrobat Pro.
  • Possession of valid California driver’s license and proof of California auto liability are required.

SALARY RANGE $60,524.00-$90,786.00 ANNUALLY
