The Health Center Nurse is a Registered Nurse who provides trauma informed care with a goal of ensuring and improving the patient health care experience. This position provides clinical support to patient care activities utilizing nursing education, nursing assessment, and application of clinical/procedural skills.
Core Job Responsibilities:
Provide health maintenance support and education for chronic diseases i.e. hypertension, hyperlipidemia, diabetes, and screenings including colorectal, mammogram, and cervical cancer screenings
Conduct Social Determinants of Health screenings to identify barriers to care and care gaps, maintain up to date knowledge of HEDIS and UDS measures, and utilize quality metrics to drive patient outreach and provide optimal care
Collaborate with Quality, Population Health, Operations, Medical Services, Health Center managers, and support staff to ensure standardization of workflows, processes, distribution of patient equipment, proper documentation, and essential follow up
Promote patient centered care model by collaborating with the patient, family, and ACCESS health care team members such as behavioral health, benefits specialist, utilization management, referral coordinators, and care coordinators
Perform nursing assessment within scope of practice
Triage patient calls and inquiries
Complete initial history and physical for new and existing patients as needed
Navigate medication prior authorizations and refill medication requests
Administer prescribed medications and treatment as directed by the provider per ACCESS policies
At the direction of Clinical Support Services, provide clinical training and clinical skills assessment verification of staff
Perform patient outreach (appointment follow-ups, abnormal labs, medication adjustments)
Provide patient care support when needed including, but not limited to, rooming, labs, vaccinations, procedures, EKGs
Perform other duties as assigned
Requirements/Preferences
Current, unrestricted RN license in the State of Illinois required
Bachelor's degree in nursing preferred
Minimum of one (1) year of experience in community or ambulatory care preferred
Current BLS for Healthcare Provider CPR certification
Strong technical skills including:
Intermediate proficiency with Microsoft Office Suite required
Intermediate proficiency with EHR required, Epic preferred
Experience using Microsoft Teams and SharePoint preferred
Bilingual (Spanish/English) and/or ability to utilize available translation services to communicate with non-English speaking patients and/or guests required

Local travel between Health Centers is required; mode of transportation required, if personal vehicle is employed, a current driver's license and proof of insurance is required
Ability to sit for up to 4 hours at a time
