Duties and Responsibilities:
Schedules all discharge appointments and coordinate services for follow up on release from facility, which includes ensuring medications transfer to the patient upon release and prescriptions are called in to a pharmacy when applicable.
Processes all necessary paperwork and reports to external provider for patient to be seen upon release.
Collaborates with the Duffy Health Center, Community Health Center and other community programs to enable transfer of health services upon release.
Serves as Volunteer Coordinator; processing clearance forms and coordinating orientation.
Develops and maintains par levels for medical and office supplies through ordering from vendors.
Coordinates the scheduling of the Health Services employees, assists with communication with the payroll department.
Works collaboratively with MA/MRC to schedule telehealth calls with internal provider for same day appointments or future telehealth visits.
Works collaboratively with MA/MRC and Human Services in Mass Health coverage and changes in medical status.
Collaborates with health services and programs team members to develop, refine, and communicate processes to support needed care.
Works collaboratively with MA/MRC to scan documents to medical records with the corresponding patient information.
Answers and responds to departmental phone calls.
Assists with processing purchase order requests for medical equipment and contractor services.
Orders medical supplies for health services.
Provides copies of medical records/information with signed release of information forms.
Works with facilities and maintenance staff for procurement of supplies and repair of equipment.
Prints reports and collects data for regulatory agencies/requirements.
Assists with projects in both Programs and Health Services departments.
Be aware of standards for ACA and DOC concerning medical records and maintain standards.

Qualifications:
A High School Diploma or equivalent certificate is required.
An Associate's Degree from an accredited college or university in a medical related concentration, office studies or health services is preferred but not required.
A minimum of two (2) years of experience working in a medical field with a concentration with scheduling is strongly preferred.
A minimum of one (1) years of experience working in a correctional facility is preferred but not required.
Or an equivalent combination of education and experience.
Work Environment:
This position does require working within the Barnstable County Correctional Facility.
This position does require working with individuals currently incarcerated at the Barnstable County Corrections Facility.
