Case Manager (Second Shift)
Full-Time/Salary
Second Shift (1:00 am - 9:00 pm)
$40,000.00 - $60,000.00/Annually
Team Recovery Ohio, LLC, the fastest-growing organization providing treatment for substance use and mental health disorders in Northwest Ohio, is seeking a Full-Time, Second Shift, Group Facilitator/Case Manager! Qualified applicants must be motivated, compassionate, and energetic, with the ability to adapt and problem-solve potential issues.
Position Summary:
The Group Facilitator/Case Manager is responsible for providing individual and group case management services, focusing on behavioral health and substance use disorders. They will work alongside the clinical team to ensure life skills improvement and long-term sobriety for clients with Behavioral Health and Substance Use Disorder Diagnoses.
Job Functions/Duties:
The essential functions include, but are not limited to the following:
- Providing support and guidance to clients that help develop and utilize daily living skills and positive coping skills throughout the recovery process.
- Coaching clients on development of healthy activities, eating habits, and the dangers of unhealthy behaviors.
- Coordinating and linking clients to community resources and/or additional services.
- Resolving conflicts that arise with clients in treatment and in recovery housing quickly, appropriately, efficiently, and in a professional manner.
- Maintaining a safe and secure environment for clients and staff.
- Driving company vehicle(s), including multi-passenger vans to transport clients to and from the treatment center, recovery housing, and other locations deemed necessary for treatment activities.
- Documenting all activities in clients electronic medical records (EMR) and filing all notes in charts daily for UR purposes.
Skills and Knowledge:
- Must possess knowledge and experience in coordinating care with clients who have behavioral health and substance use disorder diagnoses, including knowledge of Medication Assisted Treatments.
- Advanced ability to educate clients, staff, and the community on health issues and promoting overall wellness.
- Must possess conflict management and crisis intervention experience and skills, as well as possess the ability to effectively apply problem-solving skills while providing real-time solutions.
- Must possess decision-making skills and be able to collaborate with a multidisciplinary team.
- Must possess excellent time management skills with the ability to multitask, prioritize, meet deadlines, and communicate challenges effectively.
- Must possess the ability to work in a complex, detail-oriented, fast-paced organization.
- Must possess a compassionate, relationship-based, and trauma-informed approach to treatment.
- Must possess high ethical standards, and the ability to apply discretion and trust with confidential material and situations.
- Must have experience with electronic health record (EMR) systems and clinical documentation, as well as the utilization of office and clinical technologies.
- Knowledge of Gmail, Google Chat, Google Tasks, Google Docs, Google Sheets, etc.
Requirements:
- Valid Driver's License and insurance with clean driving record (required).
- Ability to commute and work at multiple sites, including the treatment center, recovery housing, and in the community.
- Minimum of 2 years of experience in Group Facilitation/Case Management (required).
- Minimum of 1 year of experience in behavioral health and substance use disorder treatment is preferred (not required).
Education:
- High School Diploma or equivalent (required).
- Associate or Bachelor's Degree in Social Work, Counseling, or LSW equivalent is preferred (not required).
- Valid CPR/First Aid, AED, BLS, and/or ALS Certifications are preferred (not required at the time of hire, but expected within the first year of employment).
- Valid CDCA and QMHS + 3 Certification (+3 years of Behavioral Health work experience) (required).
- Valid CPRS and/or CPI Certifications are preferred (not required).
