A Brief Overview:
The purpose of the Physician FPA is to provide medical direction for the Hospice patient care program according to Hospice policy and procedure. This position reports to the Vice President, Home Services. This position is 55% Health Information Technology, 25% Accountable Care Organization, 20% Medical Group practicing physician.

  • Provide excellent clinical and surgical skills
  • Connect with patients to execute quality patient care
  • Desire to teach Ophthalmology residents, interns, and medical students
  • Clinical research is also encouraged

This is an excellent opportunity to join a busy glaucoma multi-specialty Ophthalmology group with immediate ramp-up potential, and to provide quality patient care within a highly successful, expanding organization.

NorthShore's state-of-the-art Eye and Vision Center is the largest multi-specialty Ophthalmology group on the north shore of Chicago. Join a team comprised of dedicated faculty that provides a multitude of patient services in all ophthalmic subspecialties.
  • 13 Ophthalmologists and Optometrists
  • 1 Glaucoma subspecialist
  • 9 Comprehensive Ophthalmologists, most with subspecialty training.

Our supporting staff is outstanding, dedicated, and well-trained. The Medical Group utilizes Epic for medical records. Our Ophthalmic clinic equipment includes HFA units, Spectralis and Cirrus OCT, Ellex SLT / YAG lasers, PASCAL retina laser system, Optos fundus cameras, Goldmann Visual Field, and IOL Master 700. Our surgery center, located at Glenbrook hospital, is equipped with Centurion phacoemulsification machines, ORA intraoperative aberrometry system, and Zeiss Artevo microscopes with Callisto and Intraoperative OCT capabilities.
What you will need:
  • IL Medical license or pending
  • DEA
  • Board certified or board eligible in Ophthalmology
  • Required
