Join Our Team as a Geriatrics Registered Nurse!

Our team is seeking dedicated Geriatrics RNs to provide compassionate care and support to our elderly veteran population. Key Responsibilities:

• Document assessment and interventions according to policy and practice standards.
• Conduct bedside, patient-centered rounds using the SBAR tool for effective communication.
• Mentor and precept new staff and students.
• Provide crisis intervention for our unique population.
• Complete post-Emergency Department (ED)/Urgent Care and post-discharge phone calls within 2 business days.
• Actively manage a high-risk registry of patients with specific health concerns.
• Collaborate with the Teamlet and expanded Team to develop patient-driven care plans.
• Provide patient and family health education focusing on self-management, prevention, and wellness.
• Manage triage, telephone triage, and scheduled and unscheduled patient visits.
• Execute nurse-driven clinics to support positive health outcomes.
• Promote cost-effective use of resources and adopt effective non-face-to-face approaches, including Tele-Health.
• Keep providers and case managers aware of patient progress, issues, and problems.
• Screen patients for social service, home care, and community care needs; coordinate referrals as appropriate.
• Use x-ray machine, including operative C-arm x-ray machine, and wear lead protective apron.

Preferred Experience:

• Experience with operative C-arm x-ray machine.
• Strong communication and assessment skills.
• Triage experience with veterans dealing with pain issues.
• Work experience in a spinal procedure suite.
