We're looking for a Community Health Worker in Palm Beach, FL, to enhance patient health and well-being through education, care coordination, and tech support, aiming to improve community stability and reduce hospital visits. Our goal is a supportive healthcare system for seniors with mental health and substance challenges.
WHAT WE ARE SEARCHING FOR:
- Help Patients Attend Appointments: Make sure patients go to their Author Health and other healthcare appointments. Find and remove anything that stops them from using necessary resources and supports.
- Ensure Timely Follow-Up: After a patient visits the Emergency Department (ED) or stays in the hospital (inpatient hospitalization, IH), make sure they quickly follow up with their Author Health provider(s), Community Health Worker (CHW), and Primary Care Provider.
- Maintain Contact with Patients: Keep in touch with all patients you’re responsible for, as directed in their care plans. If you find new risks or needs for the patient or caregiver, let the right team member know and complete the necessary paperwork.
- Reduce Unnecessary ED/IH Visits: Find out why patients often use the ED or IH. Work together with patients to lower unnecessary visits to the ED or IH. This can include making and following a plan to avoid going to the hospital when it’s not needed.
Assess patients' social determinants of health (SDOH) needs and assist with connection to community-based organizations and agencies to alleviate those needs identified or escalate to the appropriate team member to ensure SDOH needs are met
- Work as a Team: You’ll be an important part of a team that includes different healthcare professionals. You’ll join team meetings and help with communication.
- Support Patients and Caregivers: You’ll help patients and their caregivers. This includes helping them use technology for virtual visits with their healthcare providers.
- Help Patients Navigate Healthcare: You’ll work with patients who have medical and behavioral conditions and may have trouble using the healthcare system. Your goal is to help them rely less on hospital care.
- Reduce Hospital Visits: You’ll think carefully about why patients visit the hospital and work on plans to help them avoid unnecessary visits.
- Document Care: You’ll pay attention to details and write down what care patients receive.
- Communicate Well: You’ll need to be good at working with different people in a diverse community and comfortable sharing medical information.
- Know Your Resources: You’ll need to know about community agencies and programs that can help with healthcare.
- Organize and Prioritize: You’ll need to be good at organizing resources and deciding what needs to be done first.
- Use Technology: You’ll need to be good at using technology, like electronic health records, and keep detailed, up-to-date records of patient activities.
Requirements for the Job:
- Being bilingual in Spanish-English or Haitian-Creole is a plus.
- You must have a valid driver’s license and a car.
- You need at least 1 year of experience related to this job.
- You must pass a criminal background check and be vaccinated for influenza and Covid.
- You need a quiet, private area for phone and video calls that meets HIPAA rules.
- The job requires travel to Palm Beach, FL and nearby areas. You must be able to drive and use a computer.
