A DAY IN YOUR LIFE AS A CARE MANAGER: as a professional Care Manager, you will make periodic visits to see the client in their home or facility and monitor care on an ongoing basis, including assessing medical, social, functional and psychological needs.

You will work with the family and other care team members to determine the best ways to implement quality medical care, supportive services and meaningful engagement. You may schedule and transport clients to medical appointments or other activities to advocate and coordinate follow up.

Ongoing verbal and written communications with the client and their supportive family and friends ensures that relationships are maintained and care is appropriate.

This position offers an opportunity to work from a home office with support from a dedicated administrative team. You will have flexibility in your schedule and receive backup assistance and coverage when needed.

Requirements:

  • Self-directed professional with at least two years of experience in nursing, social work or gerontology, or other related experience, preferably with older adults.
  • Bachelor’s or Master’s degree in a field related to Geriatric Care management (i.e. counseling, gerontology, mental health, nursing, occupational therapy, physical therapy, psychology, or social work)
  • Professional credentials desired, which may include LMSW, RN, LVN, LPT, Gerontologist
  • Must reside in Houston, Texas, with knowledge of Houston-area eldercare resources.
  • Excellent verbal and written interpersonal communication skills
  • Strong organizational and time management skills
  • Proficiency with general office technology

Preferred:

  • Home health experience
  • Nursing or medical social work
