Job Title: Program Coordinator- Case Manager
Reports to: Social Worker Specialist & CEO
Dept.: Youth & Family Support Services
Primary Purpose: The program coordinator performs case management for services offered to youth and families participating in Youth & Family Support Services at the Boys & Girls Club. Specific functions include intake of new clients, and tracking intervention and outcomes. They do this by completing intakes, required CANS assessment, and monitoring programs implemented at the club. Qualifications:Essential Job Duties:
- Conducts Intake of new clients
- Under the direction of the club social worker, the Case Manager serves as a liaison between clients and referral agencies.
- Completes initial intakes and screenings to identify client goals and objectives.
- Gathers data and prepares documents for interpretation.
- Ensures programs are being implemented as intended, and collects outcomes tracking tools including sign in sheets, pre/posttests, etc.
- Works closely with clients to ensure outcomes are achieved. This include phone calls, home visits, accompaniment, and referrals as needed.
- Establish and monitor adherence to policies and procedures by all program staff.
- Collaborates with other public resources to get the best results possible for each family
Knowledge, Skills, and Abilities
Basic knowledge of Family Systems Framework, Lifespan and Development. Knowledge and skill in casework methods, Awareness and ability to access community resources. Excellent organizational, communication, and interpersonal skills. Must work well with children

Job Type: Full-time
Pay: $12.00 - $15.00 per hour
Schedule:
- 8 hour shift
- Monday to Friday
- Weekends as needed
