About the role
- Position Summary: Casa Esperanza Conexiones is a 32-bed facility that provides clinical stabilization services and acute treatment services for withdrawal management to adults struggling with drugs, including heroin and prescription drugs, or alcohol. At Casa Esperanza CSS, located at the Tewksbury Hospital, we provide residential substance abuse treatment for adults in a physically and psychologically safe environment. We serve adults from Massachusetts who need residential treatment for drug and alcohol abuse, including those with co-occurring mental health issues. Our highly structured environment focuses on resolving addiction issues, building the foundation for a sustainable recovery, and developing a healthy, productive and meaningful lifestyle.
- The program is staffed by highly skilled professionals who work with each patient to develop an individualized treatment plan and determine appropriate aftercare. We are currently seeking a Registered Nurse and/or Licensed Practical Nurse to oversee the Medical Services at our Tewksbury ATS/CSS through Casa Esperanza’s Integrated Care Model, implementing doctor’s orders, monitoring specified medication/refills, monitoring and recording vital signs as needed, and maintaining medical/ nursing records. The Nurse will also observe patients and provide accurate and timely documentation of their treatment progress, and coordinate and assist Case Managers regarding client’s progress and treatment.
What you'll do
- Complete Nursing Assessments, COWS, CIWA Assessments upon admission
- Ensure monitoring of vitals and withdrawal symptoms
- Complete medical clearance exam at time of intake to establish ASAM 3.5 or ASAM 3.7 to achieve standing order including direct contact with Medical Director/ Designee
- Ensure PRN standing orders for medication interventions.
- Complete Medication Monitoring/Administration and all required documentation
- Complete Medication Counts at start and finish of shift of all control and non-control substances
- Work closely with clinical team during MDT meetings to offer and provide client medical insight as part of case presentations.
- Work collaboratively with Medical Director/Designee for medication reconciliation process
- Supervises the RN and LPN nursing team
- Coordinates weekly/monthly staff schedule
- Responsible to train and onboard new hires for medical team
- Serves as on call and back up to program and schedule needs.
- Manages compliance and quality assurance over regulatory and compliance measures.
- Directly works in collaboration with community providers for medication refills and reconciliation.
- Complete evaluations based on client request for medical attention, emergency and non-emergency triage and physical/mental health rounds for safety and condition monitoring
- Keep electronic health records up to date including progress notes and assessments of service.
- Educating clients on their current medical diagnosis, needs and medical histories; educating clients on medication information and usages
- Alerting physicians when conditions worsen or if abnormalities arise.
- Works in collaboration with counselors to ensure proper medication management and medication planning
- Works directly with Medical Director/ Designee on any medication errors and medication monitoring procedure
- Works uniformly with Recovery Specialist on medication monitoring procedure
- Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled
- Demonstrates responsibility regarding punctuality and attendance to assure staff is available to provide client care.
- Leads or co-leads client groups, as required.
- Ability to read and implement orders of the physician/designee in regards to standing orders, treatment planning
- Attend mandatory training to update knowledge base and enhance both professional and personal growth.
- Assist in developing nursing and medication records on each client
- Monitors and administers PRN medications and prescribed medication regimen in accordance with regulatory and safety standards.
- QA/QI medication and inventory needs in compliance with DPH/ DEA regulations.
- Ensure smooth transitions regarding continuity of care and medication management of patients during discharge process.
Requirements & Qualifications:
- Massachusetts Licensed Registered Nurse
- Bilingual: Spanish/ English preferred
- Knowledge of EHR systems
- Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations
- Supervision Responsibilities; as delegated per Medical Director/ Designee
- Knowledge on best practices associated from the American Society of Addiction Medicine
- Knowledge on Medication Assisted Treatment for substance use disorders
- Knowledge of Substance Use Disorder
- Compensation: $ 55 - $60 per Hour
- Holiday Pay (12 days per year + 1 floating holiday)
- Paid Time Off (PTO) - 18 days year one, 20.5 days at year 2, 23 days at year 5

The pay range for this role is:
