Description
Job Title: Foster Care Case ManagerHiring Range: $20.76-$22.63, depending on qualificationsFSLA Status: Non exempt, hourly position & 40 hours/week.This position will remain open until filled.
Position Summary
The Case Manager is responsible for the case coordination of foster children and foster parents, and for ensuring that Foster Children are receiving adequate care and are functioning to the best of their abilities within the Foster Care home and the community.

Essential Duties and Responsibilities

  1. Establish, build, and maintain rapport with children in care, foster homes, kin, and other members of the multidisciplinary team in order that optimal information gathering, and collective decision making may occur. These duties must be conducted in a manner that respects the rights and dignity of all involved.
  2. Monitor and ensure safety of foster children.
  3. Conduct in depth interviews with foster parents to ensure most appropriate services are obtained and provided.
  4. Obtain necessary and pertinent information about prospective foster child placements from appropriate agencies/individuals.
  5. Develop and maintain a clinical record for each client/family per agency standards and adhere to volume 7 regulations, ensuring files are always audit ready.
  6. Responsible for case management, including the coordination of services, case related treatment team coordination, medical/mental health, academic and all other appointments as deemed necessary for the children placed by the agency.
  7. Participate in mandatory rotation for "on call" to support our foster families.
  8. Participate in and provide input at group/individual supervision sessions to review foster children with the clinical team and the program supervisor and discuss treatment strategies and the child's progress.
  9. Have a working knowledge of community resources and refer foster children as guided by the treatment plan.
  10. Represent Ariel Clinical Services by acting as a liaison to local human service agencies and state level agencies, as required/assigned.
  11. Conduct full/update Structured Analysis Family Evaluations (SAFEs), as qualified.
  12. Assist in the process to certify Foster Care homes and place children in Foster Care homes.
  13. Participate in ongoing program evaluation to ensure program effectiveness.

Additional Duties and Responsibilities
  1. Must work closely with other Ariel Programs and offices.
  2. Performs other duties as assigned

Requirements
Required Knowledge, Skills and Abilities
  1. The ability to effectively communicate verbally and in written format in both individual and group settings.
  2. Ability to be flexible, and able to change direction rapidly.
  3. Must be available outside of traditional office hours including nights, weekends, and holidays as necessary to perform the functions of the position.
  4. Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.
  5. Basic computer knowledge.
  6. Strong knowledge of the type of work Ariel participates in.
  7. Ability to exercise independent judgment within the scope and boundaries of the program, the Agency's Standard Operating Procedures, and Federal, State and Local Rules and Regulations regarding program operations.
  8. Ability to organize and present case material in a straight-forward and concise written manner such that communication among treatment team members is clear concerning the child/family.
  9. Knowledge of basic tenets, interventions, and evaluation formats of therapeutic Foster Care.
  10. Knowledge of human development with regard to major milestones of development, normal development parameters and commonly regarded devices to measure such.
  11. Knowledge of family dynamics with regard to communication, interactions, and general functioning as well as parenting skills and intervention techniques.
  12. Knowledge of community resources (social, health, public agency, and information) in order to assist families in accessing these resources.
  13. Ability to provide reliable, insured transportation for self and others through the community served.
  14. Ability to demonstrate consistent and reliable attendance as scheduled and requested.

Education and Experience
Required:
  • Bachelor's degree in social or behavioral sciences.

OR
  • Bachelor's degree in a non-social or behavior science, and,
  • Two years' experience supervised by an individual with a MSW or other master's degree in the social or behavioral sciences.

Preferred:
  • Minimum of 1 year experience working in Child Welfare or Human Services.
  • Current Structured Analysis Family Evaluation (SAFE) certification at hire.
  • Experience in a customer service leadership role or 1 year I/DD or mental health experience.

Certifications/Licenses
Required:
  • Valid Regular Driver's License, with acceptable driving record.
  • CPR, 1st Aid, Safety Care, SIP, QMAP, etc. as outlined by supervisor.

Supervisory Responsibilities
Provides direction to Case Management Aides and Interns, as directed.
Physical Demands
1. This position requires that the incumbent be capable of walking, bending, standing, sitting, stooping, stretching, and/or reaching regularly for periods up to 2 hours without a break.
  1. This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position.
  2. This position requires lifting regularly up to ten pounds and occasionally up to forty pounds.
  3. This position requires the ability to hear, see, smell, and distinguish temperature, at a level which is adequate to ensure client and staff health and safety. Adaptive devices are acceptable.
  4. This position will require daily driving of up to 1 hour per day and occasionally up to 8 hours per day.
  5. Climbing may be required from time to time.

Work Environment
  1. Exposure to typical climate-controlled office environment when in the office.
  2. Exposure to all climates and weather conditions while working in the field.
  3. At times, physical interventions may be required.
  4. Exposure to blood borne pathogens and infectious disease is possible but limited.
  5. Exposure to hazardous chemicals is possible but limited.
