Summary of Position: The Licensed Practical Nurse is responsible for direct patient care.
Typical Hours: 8:00 am-8:00 pm, three days a week (36 hours)
Education and Experience:
1. Graduation from an approved School of Nursing.
2. Current LPN licensure in the State of Ohio and the eligibility to obtain licensure in the State of Indiana or current LPN licensure in the State of Indiana and the eligibility to obtain licensure in the State of Ohio.
3. Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
4. Hospice experience preferred.
Responsibilities:Responsibilities of Position:
- Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs including Routine Homecare Visits, End of Life/Continuous Care, Referrals, Care Center Assistance, and Chart Reviews
- Provide patient/family services according to established Plan of Care and signed physician orders
- Recognize and report promptly to the registered nurse any changes in the mental and/or physical status of the patient. Reporting should be written and oral when appropriate
- Explain hospice services and benefits in regard to Medicare, Medicaid, and Private Insurance and know when to use Palliative Care Interventions’ and ABNs
- Obtain and enter orders from Medical Directors and Physicians when deemed appropriate
- Order appropriate Durable Medical Equipment (DME) for patients
- Facilitate continuity of care by coordinating admission of home-care patients to in-patient settings (hospital, nursing home), as needed
- Work evenings and weekends as needed to meet the needs of the patients and families and agency backup schedule
- Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements
- Maintain clinical competency for assigned responsibilities and procedures
- Other duties as assigned
