MINIMUM QUALIFICATIONS:
Minimum Qualifications for GS-07: 1 full year of graduate level education or superior academic achievement OR 1 Year of Specialized Experience equivalent to at least the GS-05 grade level. Specialized experience is defined as experience that equipped the applicant with the particular knowledge, skills, and abilities to perform successfully the duties of the position, and that is typically in or related to the work of the position to be filled. Examples include: Experience in regulations of confidentiality and patient eligibility, familiarity of community resources, coordinating schedules and communicating courteously and effectively with patients and their family; knowledge of computers and programs such as EHR, RPMS and other databases. Knowledge of prevention and treatment of diabetes.
Minimum Qualifications for GS-09: Master's or equivalent graduate degree or 2 full years of progressively higher level graduate education leading to such a degree or LL.B. or J.D., if related OR 1 Year of Specialized Experience equivalent to at least the GS-07 grade level. Examples include: Experience in interpreting readings of random blood sugar, blood pressure, pulse, weight and height. Experience in motivational interviewing. Knowledge of basic medical terminology. Experience in recording data with correct grammar, spelling and punctuation. Challenging participants to pursue and sustain treatment and prevention activities which lead them towards a healthy lifestyle and self-care.

Duties
- Provides education and case management for persons with diabetes and pre-diabetes and developing processes to assist the participant with establishing a goal oriented plan for behavior change.
- Schedules appointments, tracks all visits in software applications and works closely with participants and family members to identify ways to increase exercise.
- Encourages and motivates participants to achieve specific weight loss goals, assist with behavioral reinforcement and planning and implementation of community activities.
