This position is responsible for assessing the needs of individuals and/or their caregivers by providing information and referral services enabling them to make informed decisions based on available options. Essential Duties and Responsibilities include the following. Other duties may be assigned. Client Intake and Assessment:
- Conduct initial assessments in hospital/clinic setting and/or at the client’s residence to determine needs and provide resources as appropriate.
- Develop care plans for assigned clients including formal and informal supports. Maintain daily, weekly, and monthly tracking system.
- Maintain strict confidentiality, and follow HIPAA guidelines. Attend all scheduled trainings/meetings.
- Adhere to all CICOA policies and procedures.
- Develop effective/professional working relationships with doctors, social workers, discharge planners and other interdisciplinary team members.
- Participate in daily case conferencing and daily rounds within the hospital setting.
- Must work as a member of multiple teams. Must meet CICOA departmental identified goals and objectives.
- Maintain flexibility in scheduling assessments to meet client and caregiver schedule.
- Provide education on CICOA services as well as community resources to hospital/clinic staff.
- Develop a process of referrals within the clinical setting.
- Work independently and go getter Resource Development/Management
- Accurately record time and tasks according to agency and funding source guidelines.
- Display competence for all guidelines, rules/instructions for Medicaid Waiver, CHOICE, SSBG, and Title III
- Meet and maintain on an annual basis certification requirements of the FSSA Division on Aging. Community Relations
- Develop and maintain working relationships with community organizations and service providers
- Demonstrate agency’s customer service standards

Education and/or Experience Bachelor’s and/or Master’s degree from accredited college or university in social work, gerontology, counseling, psychology, health & human services, or nursing; or a RN Licensure with one year experience in Human Services; or Bachelor’s degree in any field with a minimum of 2 years full time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring. Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole num
