Field Homecare LVN Job Summary:
- Provide in-home and virtual nursing care to patients.
- Provide support for clinical visits, including chart preparation, clinical intake, after-visit care coordination, and clinical documentation.
- Coordinate and support patient care by implementing care plans and performing outbound calls.
- Process and track orders and referrals.
- Clinical skills and procedures: Blood draws, laboratory specimen collection, medication and vaccine administration, foley catheters, wound care, removal of sutures/staples, basic hearing and vision tests, home safety evaluations, IV insertion, EKG, ABI.
Field Homecare LVN Background:
- Valid, unrestricted state LVN license.
- Requires 2+ years of direct patient care - preferably including home health, hospice or long term care experience.
- Experience working with Federal and State assistance programs (Medicare/Medicaid/etc) - work with dual-eligible members
- Must have demonstrable experience with prior field/travel assignments.
- Must have experience working with elderly, frail patient populations - patients with multiple chronic conditions
- Must be comfortable seeing members in their homes, hospice, long term care (LTC) and assisted living facilities.
- Phlebotomy experience is a PLUS
- Basic computer (MS Word & Excel) and typing skills are necessary.
- Bilingual (English and Spanish) is a plus.
- FIELD POSITION: Must be willing and able to travel throughout the assignedarea(mileage reimbursed).
