TheField Based Homecare Ambassadoris a pivotal member of theLandmarkInterdisciplinary Team. This role actively engages with patients both in the home and by telephone. The incumbent will use compassion and empathy to earn patients' and caregivers' trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships.

This position is full - time, Monday - Friday. Employees are required to work our normal business hours of 8:00 AM - 5:00 PM EST (Eastern Standard Time zone). It may be necessary, given the business need, to work occasional overtime or weekends.

Primary Responsibilities:

  • Conduct home visits and/or phone or video calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to:
  • Resource referral and connection
  • Quality measure documentation, support, and follow up
  • Telemedicine facilitation and set up
  • Collect information regarding patient conditions using standardized checklists and templates.
  • Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction.
  • Actively participate in weekly interdisciplinary team meetings

Required Qualifications:

  • High School Diploma / GED (or higher)
  • 3+ years of experience in any of the followings job categories: Social Worker (non-licensed), Home Health Aide; Certified Nursing Assistant; Certified Medical Assistant; Medication Aide; Health Coach; Health Navigator; Healthcare Technician. Candidates with experience similar to those listed here are encouraged to apply as well
  • Reside OR Live within 1 hour commutable distance from Newnan or Douglasville, GA
  • Ability to conduct independent home visits up to 75% of the timeOR have experience conducting home visits
  • Knowledge and proficiency of basic Microsoft Word, Microsoft Excel, and Microsoft Outlook applications
  • Access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy
  • Ability to work an 8-hour shift between the hours of 8:00 AM - 5:00 PM EST
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Preferred Qualifications:

  • Geriatric or primary care experience in institutional settings of care such as long-term care/skilled nursing facilities preferred facilities

  • Superb written and oral communication skills and comfort level with technology

Physical and Work Environment:

  • Regular requirement to sit; use hands to touch, handle or feel.
  • Regular requirement to talk and listen.
  • Occasional requirement to stand; walk and reach with hands and arms
  • Occasional requirement to lift and/or move up to 10 pounds.
  • Vision abilities include close vision, peripheral vision, depth perception and ability to adjust focus.
  • Ability to travel frequently by car
