MAJOR FUNCTION: Conduct outreach to the Migrant season farmworker population where they live, work, and congregate, as well as the agricultural industry and agencies that support the needs of the MSFW program. Build institutional relationships and maintain existing relationships with community-based groups that serve the needs of the migrant community. The Farmworker Health Specialist works with the team to plan and coordinate visits to outreach camps, ensuring that all camps are visited promptly, and all necessary follow-up visits are completed. Provide transportation for patients in CompleteCare vehicles.
Hours: Seasonal position (Part-time). Hours may vary but are generally afternoons, evenings, and weekends. This position is seasonal, with an end date of October 1st.
QUALIFICATIONS:
- Strong analytical, presentation, communication, and problem-solving skills.
- Two years of related experiences, such as community services, the healthcare environment, or social services work
- Good organizational, verbal, and written communication and multi-task skills are essential.
- Must be able to communicate effectively in Spanish.
- Certification: CPR a Plus
- It is expected that this position will drive a company vehicle to transport patients and perform outreach duties, so maintaining a driving record in compliance with CompleteCare Health's insurance policy is a requirement.
ESSENTIAL RESPONSIBILITIES:
- Conducting outreach visits to farm labor camps in Southern New Jersey Gloucester, Cumberland, and Cape May County.
- Submit to Manager every week program data and patient satisfaction surveys (May to September)
- Responding to phone calls from farmworkers
- Assist with program operation as needed by conducting outreach at migrant camps, community events, and other community venues; conducting individual and group health education sessions; serving as a liaison and/or interpreter between farm workers and the health center staff; providing transportation to CCHN locations for patient appointments.
- Perform health screenings.
- Assist in coordinating migrant patient care visits, follow-up calls, and other healthcare needs.
- Plan and promote outreach events at multiple sites, including farmworker appreciation events and farmworker health fairs.
- Responsible for insurance verification into Sage and scanning and collecting documents needed.
- Meet monthly goals set in the department.
- Register all migratory and seasonal agricultural workers in the electronic medical system and update current patient demographic and insurance information and documentation daily.
- Assist all migratory and seasonal agricultural workers who need help with completing any necessary paperwork.
- Must be able to work flexible hours, as evening and weekend work are necessary.
- Ability to work with teammates in a professional and solutions-oriented manner.
- Assists in clinic set-up, closure, and cleaning procedures.
