PROGRAM STRUCTURE: The Families Moving Forward (FMF)- FRSP program will provide each family housing assistance in the form of assistance with locating an appropriate unit (meeting rent reasonableness standards and pass inspections by DHS, Subsidy Assistance for an initial period of 12 month period. Initial financial costs will be paid using Housing Prevention and Rapid Rehousing Program funds (HPRP). The families will go through program Re-certification every 3 months to assess the continuation or termination from the program up to 18 months. Head of households will be required to increase income by 60%, to be able to sustain housing without financial assistance.
Description: The Family Support Worker shall coordinate all Provider types (including Secondary Service Providers, District Agency Providers, and Community Providers) services and ensure that families make meaningful progress toward increasing earnings and achieving housing stability. The FSW shall be assigned a minimum of 17 families and a maximum of 22 families at any point in time.
The FSW shall be responsible for conducting outreach to referred families and administering the 2Gen approach, engaging families in Provider activities, re-engaging those families who started to participate and subsequently stopped, introducing families to Provider processes, reviewing DHS-administered assessments, developing, and amending housing stabilization plans, and helping families overcome barriers to program engagement and participation. FSW shall develop trusted working relationships with assigned families and shall monitor families’ engagement and progress for as long as the families are in the program. FSW shall coordinate multiple activities outlined in the housing stabilization plan. FSWs are ultimately responsible for the engagement of families and should work with TEP Providers to identify appropriate education and occupational training programs as well as employment opportunities for families.
Qualifications
- · The minimum qualifications bachelor’s degree in Social Work, Psychology, Sociology, Counseling, or related service/science or health care related disciplines and one (1) year of experience providing case management services (internship experience applicable).
- Without a bachelor’s degree can meet minimum qualifications by substituting at least two (2) years of work experience in a social service and/or human service field. Certification and/or licensure in relevant discipline (e.g., certified addictions counselor) may also substitute for educational requirements.
- Demonstrate interpersonal and effective communication skills (required)
- Demonstrate effective time management skills & detail oriented (required)
- Ability to obtain a clear local and FBI police clearance, drug testing and child abuse registry clearance (required)
- Must be able to follow confidentiality protocols (required)
- Valid Driver’s license and working vehicle (required)
- Preference will be given to qualified DC residents.
OTHER REQUIREMENTS: Valid driver’s license, copy of driving record, and proof of valid auto insurance; FBI Clearance, local police clearance, DC and local Child Abuse Registry Clearance; TB Screening; Drug and Alcohol Screening
PHYSICAL REQUIREMENTS: This position requires the ability to speak, hear, see, and lift small objects up to 20 lbs. May require assistance with our FMF Food Pantry, or Diaper Village and navigate stairs, etc.
