ASSERTIVE COMMUNITY TREATMENT (ACT)
FAMILY NURSE PRACTITIONER
Position Summary: The ACT Family Nurse Practitioner will provide Nurse Practitioner services as scheduled to assigned caseload including assessment and prescription medication on and off site. This position is community based and is a full-time position.
Job responsibilities:
- Take part in all agency-sponsored training programs required by supervisor.
- Provide Nurse Practitioner services as scheduled to assigned caseload including assessment and prescription of medications to improve or maintain a member’s wellness.
- Participate as required in team meetings to develop treatment plans and strategies for specific clients.
- Collaborate in providing services to persons in acute or chronic health needs that are exacerbated by their mental instability, living habits, lifestyle choices, genetics, and other complicating factors.
- Positive role modeling and communication is respectful and culturally competent.
- Ensure that documentation is timely, accurate, and reflects a high professional standard.
- Carry out duties at sites assigned and, by agreement, for other agencies by contract to LSC.
- Using the electronic medical records, input data and following through seeing services to their completion and provide all necessary data for billing.
- Maintain ethics, legal aspects of clinical practice, and professional standards, including boundaries.
- Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards and Maryland State Regulations.
- Other duties as assigned by the ACT Director.
Qualifications, Education and Experience:
- Formal training as a Family Nurse Practitioner with license and insurability in the state of Maryland.
- Current CPR and First Aid certification; driver's license and safe driving record.
- Excellent verbal and written communication skills.
- Demonstrated ability to work with professional and non-professional colleagues effectively and to provide clinical supervision to health professionals in a teamwork environment.
