Job description
- Schedule: M-F 8am to 5pm with every 5 weeks call - phone calls only and are limited
- Worksite Setting: Hospital owned outpatient center
- Scope of Work
- Patient Population: Adult coverage must see 13+ years and up.
- 20% geriatrics, 80% adult medic and around 5% Medicare / Medicaid.
- Support Staff: 1 clinic administrator, 1 APP, 2.5 clinic support and 1.5 clerical support
- We currently serve patients with chronic metabolic conditions, primary care, management of chronic disease prevention, care for minor illness and injuries, and health care maintenance utilizing consultations and community resources when available and appropriate. Usual diagnoses/conditions treated are thyroid disease, high blood pressure, high cholesterol, diabetes mellitus, obesity, upper respiratory infections, urinary tract infections, malaise, fatigue, anxiety, depression, chronic obstructive pulmonary disease, chest pain, Gerd, and preventative visits such as cancer screenings. Point of care testing and lab services are provided on site.
