*We currently have an opening for two Family Case Managers under different grants - one for Parents as Teachers and the other for Healthy Families America. Both are full time positions*
The Family Case Manager is accountable directly to the Home Visiting Program Manager. This position provides home visitation services and ensures home visitation activities meet requirements of the DC Health grant and the Healthy Families America evidence based home visiting curriculum. The Family Case Manager is responsible for providing advocacy and support services to the families to promote maternal and child health, reduce toxic family stressors, and promote strong parent/child bonds. Activities will include recruitment, developing Family Support Plans, supporting families in achieving health and stability, coordinating appropriate referrals, and integrating the family with other community resources and agencies. This is a full-time position.
The Family Case Manager is accountable directly to the Home Visiting Program Manager. Must provide home visitation services and ensures home visitation activities meet requirements of the DC Health grant and the Parents As Teachers (PAT) evidence based home visiting curriculum. Must Utilizes holistic approach to promote positive parent-child interactions and healthy childhood growth and development. This is a full-time position.
Highlighted Duties and Responsibilities for PAT:
  • Implement the PAT model to fidelity with families
  • Develops Family Support Plans that are updated at least monthly and signed by clients and support worker, which incorporate goals for both adults and other children in the family if appropriate.
  • Meets with families on case load face to face in the home environment to review progress on Family Support Plan, and to follow-up on tasks and services. Frequency of visits are based on PAT standards.
  • Networks with COH and other community resources for housing, employment, substance abuse treatment, mental health treatment, training, child and family services, etc., and makes referrals as appropriate. Progress on referrals provided, to include follow up, should be documented in the DCRS database
  • Completes required assessments and screenings with families at designated intervals, e.g., ASQ-3, ASQ: SE-2, PHQ-9, Adult Adolescent Parenting Inventory, and the Abusive Behavior Inventory. All completed assessment should be scanned in the DCRS database.
  • Meets weekly with the PAT Program Coordinator for 90-minute reflective supervision sessions. Communicates about client and case management issues in person, via e-mail, or by phone with supervisor as needed outside of regularly scheduled meetings.

  • Meets with families on case load face to face in the home environment to review progress on Family Support Plan, and to follow-up on tasks and services. Frequencies of visits are based on HFA standards.
  • Networks with COH and other community resources for housing, employment, substance abuse treatment, mental health treatment, training, child and family services, etc., and makes referrals as appropriate;
  • Completes required assessments and screenings with families at designated intervals, e.g. Parent Survey, ASQs, PHQ, etc;
  • Meets weekly with the Family Case Management Coordinator for 90 minute reflective supervision sessions. . Communicates about client and case management issues in person, via e-mail, or by phone with supervisor as needed outside of regularly scheduled meetings;


Minimum Qualifications:
  • A.A. or B.A in Human Services field (social work, early childhood education, family studies, or psychology) or equivalent professional experience preferred. A high school diploma is required.
  • For Bachelor’s of Social Work degree or Master’s of Social Work degree holders, the appropriate DC license must be obtained prior to start date
  • Experience providing public health education (ex. reproductive health, smoking cessation, child development, and/or parenting) to at-risk populations.
  • Demonstrated cultural competence in communicating with underserved, diverse populations.
  • Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required.
  • All COH staff must be vaccinated against COVID-19 and Booster (when eligible) prior to starting. We will accept religious or medical exemptions applicable by law
