Job description

Do you want to positively impact the lives of foster youth 17-21 years of age in Pinal County ? In the role of an Extended Foster Care (EFC) Success Coach you will provide support and services to assist current DCS enrolled young people in strengthening their protective and promotive factors, developing and maintaining stable living situations, increasing employability, and increasing educational attainment. In support of the young person's identified priority goals, you will alo assist in connecting them to personal and community resources, to informal support networks and life skills development in accordance with DCS contractual service requirements. This role is critical in providing young people case management and interventions as needed for youth and young adults preparing for independence in a manner that is culturally responsive, strengths based and trauma informed. This position performs responsibilities and provides effective support to young people with regular supervision, guidance and direction and is available to serve young people at times most convenient for them, which may be outside of regular work hours. What You'll Do:

  • Build and maintain a supportive relationship with youth/young adult that assists them to develop goals and a plan for achieving those goals. Maintain contact with young adult(s), supportive connections and community partners to assist and support the development and progress towards the young person's identified needs, per contractual service requirements until completion of Extended Foster Care.
  • Effectively and collaboratively monitor young person's transition to adulthood, providing support and resources in alignment with the priority needs to the young person and their team, including but not exclusive of: obtaining, building and maintaining permanent connections to caring adults, cognitive and social-emotional competencies (life skills), opportunities for educational programming, employment, housing and person-centered transition planning.
  • Attend and participate in all required meetings, including at the team, department, agency and client level, including participating in and/or facilitating team-based planning meetings at the invitation of the youth such as wraparound meetings, family engagement meetings, and permanency or community roundtables.
  • Provide individual support and services focusing on activities to develop the young person's protective and promotive factors utilizing Youth Thrive framework, Family Finding, and 3-5-7 Model.
  • Provide support to assist in crisis stabilization by connecting young person to support services and assisting with interventions to address immediate safety until the young adult can start or continue in goal progression. Work with young person to identify causes and plan for future stabilization. This may require working outside of traditional working hours, depending upon the need of the young person.
  • Maintain accurate client records, completing case notes and supplemental documentation in accordance with established timelines, program and contract standards.
  • Complete and provide required monthly, quarterly and annual data reports in accordance with established program and contract standards.
  • Maintain professional boundaries and interact professionally, clearly and timely with internal and external customers and community professionals. This includes presenting in a professional manner to all internal and external meetings with young adults, team members and external community and stakeholders.
  • Act as a liaison with community service providers by consistent and open communication and collaboration. Coordinate with service providers to ensure that the youth/young adult's needs are being met. Develop linkages and collaborate with community service providers and other key stakeholders.
  • Represent AzCA as a trauma informed agency while staffing cases, participating in meetings, providing consultation and advocating on behalf of young people.
  • Maintain consistent attendance and punctuality to provide consistency in serving clients.
  • Be available outside of normal business hours to provide client services.
  • Perform work and represents AzCA in accordance with our mission, vision, values and Foundations for Success.
  • Perform other duties as assigned, which are consistent with the general responsibilities of the position.

Minimum Education & Experience:

Relevant Bachelor or Associate's Degree and 2 years working directly with youth/young adults 14-26 who are involved in systems of care, which may include Foster Care, juvenile justice, runaway and homeless youth programs.

Staff with lived experience in systems of care, which may include Foster Care, juvenile justice, runaway and homeless youth programs is preferred.

Relevant Degree includes the following degrees: Social Work, Psychology, Counseling, Marriage and Family Therapy, Behavioral Health, or Education (or equivalent college credits to equal an Associate's degree or higher).

    Minimum Qualification & Requirements:

    • Must be 26 years of age (contract requirement)
    • Possess and maintain a valid Arizona driver's license and be insurable under the agency's automobile policy. This includes no "at fault" or criminal violations, nor more than six (6) points accumulated in the most recent thirty-nine (39) months.
    • Must be able to provide own transportation to and from clients' homes and/or other relevant organizations/agencies.
    • Must be able to provide DPS fingerprint clearance.
    • Must be able to meet training and agency compliance requirements for the position.
    • An individual may qualify for this position if the individual is able to "perform safely" the essential functions of the position with or without reasonable accommodations. Perform safely means to perform without causing a direct threat to the health or safety of the individual or others that cannot be eliminated by reasonable accommodations, as defined by 42 U.S.C. #1211 (b) and the Commission's regulations found at 29 C.F.R #1630.2 (r).
    • Must be available to work evenings and weekends to accommodate client schedules. An individual may qualify for this position if the individual is able to "perform safely" the essential functions of the position with or without reasonable accommodations.
