Job description

The Escalation Coordinator performs duties related to increasing the relationships with community stakeholders including youth and families who are requesting assistance in accessing Multi System Youth waiver and/or Prior Authorizations for Psychiatric Residential Treatment. The Escalation Coordinator will remain focused on increasing the skill level of wrap facilitators while also directly working with some families. The Escalation Coordinator assures care is delivered in a manner consistent with strength-based, culturally competent, family driven, and youth guided values, offers consultation and education to staff and providers regarding the values and implementation of the model, monitors progress toward treatment goals and assures that all necessary data for evaluation is gathered and recorded. The incumbent will support and uphold a commitment to racial and ethnic equity and antiracism.

Essential Duties and Responsibilities:

  • Works with clinical director and/or supervisors and/or practice support specialists to ensure adherence to wraparound values, the practice model and to ensure effective multi-system decision making processes in all functions of child and family teams.
  • Uses referral information, CANS assessment and other data to complete strengths-based assessment to compose the family story for use by child and family team as they collaboratively develop a plan of care with clearly defined goals.
  • Engages with Child and Family Team at the request of a Prior Authorization for a PRTF placement. Completes the Prior Authorization through completion including obtaining additional clinical documentation and sharing with identified providers by the PRTF Placement Team. Continues to manage youth in PRTF placement to discharge, completing transition to ongoing Care Coordinator at appropriate time.
  • Engages with Child and Family Team at the request of an out of home placement and/or need for Multi System Youth waiver application. Completion with assigned Care Coordinator of the MSY application. Tracking the monthly updates of the MSY invoices and tracker to ensure that invoices and updated applications are submitted appropriately and timely with the assigned Care Coordinator.
  • Works collaboratively with local coaches and practice support specialists to develop strong skills in relation to the phases of wraparound and models strength based, culturally competent, family driven and youth guided practice.
  • Collaborates with clinical director and/or supervisors, and/or other lead wrap facilitators in managing flow of cases, including enrollments, assignments, transfers, and disenrollment.
  • Models for wrap facilitators how to manage service utilization within the financial parameters of the case rate or other established financial protocol.
  • May assist with chart audits.
  • Willingly completes other duties as assigned to advance the mission of Choices.

Qualifications:

  • Minimum of bachelor’s degree in social work or related human service field or comparable experience in the human service field.
  • Minimum of eighteen months employment as a Choices Wrap Facilitator.
  • In the states where applicable, demonstrated knowledge regarding requirements for the wraparound certification process. Certification in wraparound preferred.
  • Demonstrated skill in leadership, fiscal management activities, team building and staff development.
  • Strong knowledge of and genuine respect for youth and adults with mental health issues and a firm commitment to empowering their families.
  • Strong communication and writing skills.
  • Highly organized, detail oriented and can effectively manage multiple priorities simultaneously.
  • Demonstrated ability to work effectively with internal and external individuals, including other professionals in the community.
  • Work effectively as a member of a team.

Salary: $53,000 annually
