Job Description
  • Completes all enrollments for the project to place in immediate shelter in coordination with the county 
  • Sets up hotel reservations for all clients that are to be enrolled into the program
  • Ensures all intake and enrollment documents are complete and accurate
  • Collects all client documents that the client has at time of enrollment
  • Assists clients with setting up transportation (Uber/Lyft, bus passes) to the hotels once enrollment is complete
  • Enters all data into the company database and HMIS, enroll all clients into the Coordinated Entry System
  • Creates new client file that includes all intake and enrollment documents required by the contract Statement of Work (SOW)
  • Provides clear overview and expectations to the client of program requirements
  • Identifies any apparent client barriers at the time of enrollment and communicate through an enrollment summary
  • Assists housing navigators and supervisors with completing client exits
  • Ensures that all necessary steps and documentation are complete and in accordance with contract SOW and program processes and procedures
  • Ensures all exits are completed in company database and HMIS
  • Assists with onsite exit process when needed
  • Identifies new potential hotel partners and reach out to secure a partnership

  • Completes new Hotel agreements with partnering hotels when they agree to partner and annually when time for renewal
  • Enters all new hotels into company database and make updates as needed when information changes
  • Communicates new hotel agreements to the team
  • Maintains hotel relationships by ensuring regular communication and check in with hotels
  • First point of contact for hotels when there are issues or programmatic changes or questions
  • Advises leadership of any concerns that arise
  • Provides support to the housing navigator team by covering case management when staff are unavailable or there are vacant caseloads
  • Attends team meetings as scheduled
  • Completes any necessary reporting needed for program duties
  • Communicates with the program team and county partners
Qualifications
  • College degree in Public Health or Social Service preferred but not required
  • Two to three years of experience in a Social Services field working with diverse populations preferred but not required
  • Ability to work with little or no supervision
  • Proficient computer skills in Microsoft Office, Email, and Data Entry
  • Technical savviness with technology tools and resources
  • Strong communication skills both written and verbal
  • Ability to travel throughout the County
  • Ability to work with diverse populations including those experiencing homelessness
  • Ability to maintain confidentiality
  • Understanding of Trauma Informed Care, Motivational Interviewing, and Harm Reduction
  • Ability to encourage and promote positivity to clients through motivational interviewing
  • Ability to interact with various internal and external teams: internal peers and departments, county representatives, property owners/managers, hotels staff, various agencies, and non-profits
  • Strong awareness of encrypting sensitive information when communicating client information
