Job Role Summary
This position plays a pivotal role in helping diagnose, treat, prevent, and respond to end the HIV epidemic. This position works under the Ryan White Ending the HIV Epidemic (EHE) grant program and working as part of a team will be responsible for locating and linking HIV reactive individuals that are lost or not currently in care. This position will collaborate with clients to increase their knowledge about HIV disease, as well as inform them about the Indianapolis Grant Area's continuum of care and the steps necessary to begin receiving services under this program. This position is responsible for planning, implementing, and providing HIV health education, risk reduction, wellness education, and retention in care within the HIV community. A portion of the position's time will be focused on reducing barriers to care for communities of color, and priority populations who are living with HIV. This position will be funded through HRSA Ryan White Grants, and continued employment is contingent upon receipt of the grant.
Associated Job Duties

  • Assesses care information to establish investigation priorities.
  • Utilize multiple data ports to assist in locating and linking HIV patients into the care and treatment system.
  • Maintain confidential case records and ensure that they are completed and disseminated accordingly. Manages a list of presumptive HIV positive individuals
  • Educates partners, support networks, and clients living with HIV about transmission and how to reduce risk of HIV and STI transmission.
  • Provides a variety of HIV prevention presentations to the identified populations, service providers, communities, professionals, and other entities that are responsible for the care, financial allocation, or advocacy of enrolled or eligible clients.
  • Works to reconnect out of care clients to HIV case management and treatment services.
  • Assist in the development of program objectives that prioritize those in the community most in need of care, support, training, and skills development.
  • Completes necessary reports and forms, and maintains thorough documentation of all interviews and field activities
  • Works with agencies that provide HIV Counseling and Testing to provide a link between initial HIV diagnosis and entry into HIV care and services.
  • Navigates and monitors High Risk HIV positive clients through the TGA's continuum of care.
  • Writes, reviews, and edits comprehensive reports that document the overall success, challenges, outcomes, and trends of the Outreach, Linkage, and HE/RR Program.
  • Performs phlebotomy and other specimen collection procedures in the field and in a clinic setting for patients in need of testing
  • Assists in the mobilization of community partnerships to identify and solve health problems related to the populations served.
  • Assists in the development of program objectives that target those in the community most at risk and/or most in need of care, training, and skills development.
  • Participates in public health emergency preparedness exercises and response to public health emergencies, as directed.
  • Participates in two H.O.P.E. events per year and health fairs. Serve on appropriate committees and task forces as assigned.

Qualifications
  • Completion of a Bachelor's Degree in Public Health, Social Work, Political Science, Education or other Social Sciences, and allied health field is preferred; or three years related experience and training; equivalent combination of education and experience.
  • Experience in providing outreach and health education services.
  • Knowledge of HIV/AIDS and/or STI related issues is beneficial, but not required.
  • Valid Indiana driver's license

Knowledge, Skills & Abilities
Language Skills:
  • Excellent interpersonal, oral, and written communication skills
  • Ability to read and interpret documents such as medical information, laboratory results, public health laws, safety rules, operating and maintenance instructions, and procedure manuals.
  • Bilingual applicants are a desire (i.e., Spanish, but not a requirement of employment)
  • Capacity to work with patients and staff regardless of age, race, gender, sexual orientation, and /or religious affiliations
  • Basic computer skills including the ability to utilize database, spreadsheet, webinar, power point, and word processing software.
  • Establishes and maintains effective lines of communication and collaboration with the community, targeted populations, and other program areas within the MCPHD.

Reasoning Ability:
  • Ability to analyze data to identify problems and to devise and implement corrective actions.
  • Ability to solve practical problems and deal with a variety of complex situations in short time frames.
  • Strong organizational skills, including attention to detail, multi-tasking, establishing workload priorities, and meeting deadlines.
  • Ability to work independently and in team relationships effectively.
  • Supports the MCPHD vision and mission through department presence in the community.
  • Performs other duties, as assigned.

Working Environment
  • Standard office environment.
  • Field work that includes engagement with clients, consumers, and service providers.
  • Exposure to communicable disease, illness, and irate community members.
  • Some physical effort associated with filing, carrying supplies for health fairs and/or exhibits, travel between main office and meetings, program site(s), and health fairs.
  • Required to drive both in-county and out-of-county for the coordination of program initiatives.
  • May be required to work evenings and weekends.
