DDS/DMD degree
Endodontic Certificate from a university accredited by the Oregon Dental Board
Active Oregon State Dental License
Active Malpractice Insurance
DEA and CPR Certification

We are a high producing fee-for-service and PPO private practice. Compensation from $1,000.00 per day or 50% adjusted production. DOE
