As the Ending HIV EpidemicCommunity Health Worker (EHE CHW) in Miami-Dade, you will blend the mission of AHF with patient access and engagement. Your goal will be to assist in keeping patients in care. Within your duties of HIV testing, you have the opportunity to engage in outreach within the community and educate those at risk. You will support clients who are at risk of non-adherence. You will assist them in navigating the healthcare system and achieving positive healthcare outcomes. Utilizing your own community resource experience and a strong skill set in understanding and working through the healthcare system, you will work with new patients and at-risk members. Your compassion and ability to relate will comfort and trust the patients with whom you provide this care, engagement, and access to resources.
Combined with a thorough understanding of the EHE Guidelines/Standards in Miami-Dade, you will collaborate with the Medical Case Management team, the Healthcare Center Provider(s), and team and outside resources so each patient receives access to available resources and the support required to be successful with their medical care.
Your strong experience in understanding the locality and the specific challenges facing this population to find creative solutions for re-engaging members at risk of or have been lost to care will be pivotal in this role. Keeping patients engaged in and understanding their healthcare is the goal of the EHE Community Health Worker so that healthy behaviors are achieved, resulting in a healthier population. With your creative skillset, you will work to achieve stronger engagement and health outcomes. As an EHE Community Health Worker, you will contribute to Managed Care's success in achieving the Managed Care Missions of Your Health.... Our Mission and the overall mission of AHF.
