Job Description

Provides employment data collection for reports, funders, and Development for the Supportive Employment Program and under the guidelines of funding and monitoring entities. Maintains data base and inputs ongoing service provision for clients served in Client Electronic Form. Provides ongoing Employment information and service provision in meetings and community opportunities.

Review and maintenance of required employment productivity and billing activities through Care Logic System including, but not limited to: Data entry, developing reports, review, and update of any forms, addressing data input with staff, and assisting staff in navigating the Care Logic System.

Compiles, prepares, and maintains employment files, keeping accurate records and completing progress note forms and other forms that may be required by circumstances described in Daybreak's Employment Manual and as required by funding and monitoring entities as well as evidenced based practices that are being conducted.

Responds to referrals by integrating them into the Care Logic system and assigning an Employment Specialist.

Assists in facilitating and leading Employment/Education Development Services and Employment/Educational Skills Training per the Supportive Employment Programming and any Evidenced-based strategy utilized in programming.


Reports and assists in review and effectiveness of Supportive Employment Program services and evidenced-based practices, when needed.


The scope of services is in-house and can and will be conducted in community and off-site areas including, but not limited to: In-reach facilities, libraries and other public spaces, homes/apartments when appropriate and necessary.

Provide appropriate crisis intervention in situations of client crisis, client conflict or in other situations where individual or group intervention advances client's progress toward treatment goals which may include maximizing the resources of current staff, and potential outside professionals.

Provide timely documentation and review of progress notes, billing, and file review through Care Logic.

Maintains the Care Logic system for the Employment Initiative.

Implements and reviews ongoing assessments, employment tracking forms and necessary CARF documentation for each individual participant, as necessary.

Assist in compiling data from progress notes, billable services, and employment plans for the purpose of reports and progress of individual participants.

Assists with developing, implementing and follow through with appropriate internal and external activities appropriate for participant engagement.

Works to provide a relationship with community employers, when appropriate and necessary.

Assists in working with other agencies and work sites, as assigned.

Assists in developing, conducting, and facilitating creative employment activities, as assigned.

Maintains the cleanliness and organization of the employment space including maintaining the safety and security of all equipment, materials and/or tools.

Assists and promotes client progress through various means and ways that are appropriate, reflective, and acceptable for the clients.

Maintains limits using natural and logical consequences as described in the Employee Manual and as required by monitoring and funding entities.

Utilizes the therapeutic milieu model to assist in managing the behaviors of self and youth.

Provides referral, outreach, advocacy, and linkage with educational, employment, creative and financial resources internally and externally as assigned and needed.

Maintains communication with Supportive Employment Program Director, Case Managers, and participants' program sites (Drop-In, Shelter, Residential, Housing, external services, etc.) regarding youth's participation and/or progress.

Attends training/agency meetings, as scheduled. Responsible for completing all required trainings needed to comply with Daybreak policy. Maintains appropriate in-service training hours by attending conferences, workshops, and training that are relevant to the job description and assignment tasks.

Participates in outside agency professional development opportunities, collaboratives, meetings and/or relative boards as appropriate, available, and accessible in conjunction with the scope of their responsibilities.


It is the policy of Daybreak to ensure that the facility is in compliance with all license requirements and that all employees are fit for duty. Once a conditional offer of employment has been given, Daybreak requires that all prospective employees sign a background check release form and pass pre-employment testing before a start date may be given.

Background Check

Daybreak is required by State of Ohio law to perform a criminal background check, which includes fingerprinting, of eligible applicants. In addition to the fingerprints, Daybreak requires the following additional background checks.

  • Federal Exclusion databases including:
    • National Sex Offender Registry
    • Office Inspector General (OIG)
    • Nurse Aide Registry
    • SAM
    • of Developmental Disabilities (DODD)
  • Motor Vehicle Report

Pre-employment Testing

The following pre-employment tests will be conducted at Daybreak's expense. Tests include:

  • A tuberculin skin test or chest x-ray (by a physician of Daybreak's choosing)
  • A urine drug screening (by a testing facility of Daybreak's choosing)

Should the pre-employment reviews indicate that the prospective employee does not meet the employment requirements, Daybreak has the option of either not hiring the person or postponing employment until the condition prohibiting employment has been removed.

If the background or pre-employment checks contain information that the applicant has committed, been convicted of, or pleaded guilty to any criminal offense, unless the charge was a minor misdemeanor not involving violence or a sexual offense at least 5 years old, or if the applicant refuses to submit fingerprints for a criminal background check, the conditional offer of employment will be withdrawn immediately.

Pursuant to SB 38 (child-serving agency), if it is determined that the applicant has committed one of the disqualifying events listed on the attached Ohio Revised Code Disqualifier sheet may prevent employment at Daybreak.
