Employment Case Manager

Description: The Employment Case Manager will work toward assisting members gain employment by coordinating the provision of treatment and support services.

Responsibilities:
- Conduct assessments to determine client needs and develop appropriate care plans
- Coordinate care, transport members to appointments
- Monitor client progress and make adjustments to care plans as needed
- Refer to Counselor as needed
- Assist clients in accessing community resources and support programs
- Utilize crisis management techniques to address urgent situations
- Collect and maintain accurate data on client progress and outcomes

- Teach skills necessary for employment (customer service, professionalism, healthy conflict resolution)

Experience:
- Bachelor's degree from an accredited institution in related field (e.g., Social Work, behavioral Health or Human Services)
- Experience working in behavioral health or a related field preferred
- Knowledge of working with individuals with Mental health Diagnoses and individuals recovering from Addiction is helpful
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Excellent organizational and time management skills

Job Type: Part-time

Pay: $18.00 - $20.00 per hour
